“逆向核对法”减少静脉药物配置中心用药差错的实践

摘　　要：目的探讨“逆向核对法”对于静脉药物配置中心减少用药差错的实践效果。方法以甘肃省某三甲医院静脉药物配置中心调剂、核对、配置等多环节用药为研究对象,比较应用“逆向核对法”实践前后调剂、核对、配置等多环节用药差错次数。结果调剂、核对、配置多环节用药应用“逆向核对法”可减少差错的发生,提高用药准确性。结论“逆向核对法”能有效显著减少用药差错,提高用药准确性,具有一定的推广价值。Objective To study the role of the“reverse check method”in reducing the incidence of medication errors in a pharmacy intravenous admixture services(PIVAS).Methods Taking dispensing,check and admixture as indexes to be investigated,the numbers of medication errors related to dispensing,check and admixture before and after the adoption of the“reverse check method”were compared.Results The application of the“reverse checking method”could reduce medication errors and improve the accuracy of medication.Conclusion The“reverse check method”can effectively reduce medication errors,improve medication accuracy and is to be made more accessible.
