超声多模态评分对复发性自然流产患者子宫内膜容受性的评估价值

摘　　要：目的:研究在复发性自然流产患者子宫内膜容受性评估中应用超声多模态评分的临床价值。方法:选取2021年6月至2022年6月到佛山市妇幼保健院就诊的复发性自然流产患者100例作为观察组,并选取同期无自然流产病史且已生育的育龄女性100例作为对照组。对两组受试者均进行激素水平检测及不同模式的超声检查,获得子宫内膜的超声多模态评分,并对其激素检测结果、子宫动脉血流参数及子宫内膜超声多模态评分结果进行对比分析。结果:两组受试者血清孕酮、促黄体生成素、促卵泡生成素、雌二醇、睾酮、催乳素的水平对比差异无统计学意义(P>0.05)。观察组患者子宫动脉的血流搏动指数、阻力指数、收缩末期峰值与舒张末期峰值比值均高于对照组女性,差异有统计学意义(P<0.05)。观察组患者的子宫内膜超声多模态评分低于对照组女性,差异有统计学意义(P<0.05)。结论:复发性自然流产患者的子宫内膜超声多模态评分显著低于正常育龄女性,因此可利用子宫内膜超声多模态评分评估复发性自然流产患者的子宫内膜容受性,以避免因人为因素影响临床诊疗,保证诊疗结果的客观性,临床价值显著。Objective:To investigate the clinical value of ultrasonic multimodal scores in endometrial receptivity assessment of patients with recurrent spontaneous abortion.Methods:100 patients with recurrent spontaneous abortion who visited Foshan Maternal and Child Health Hospital from June 2021 to June 2022 were selected as the observation group,and 100 women of childbearing age who had no history of spontaneous abortion during the same period were selected as the control group.Hormone level detection and different modes of ultrasound examination were performed in both groups to obtain the ultrasonic multimodal score of endometrial,and the hormone detection results,uterine artery blood flow parameters and the results of endometrial ultrasonic multimodal score were compared and analyzed.Results:There was no significant difference in serum levels of progesterone,luteinizing hormone,folliclestimulating hormone,estradiol,testosterone and prolactin between 2 gr oups(P>0.05).The blood flow puls ation index,resistance index,ratio of end systolic peak to end diastolic peak of uterine artery in observation group were higher than those in control group,and the difference was statistically significant(P<0.05).The ultrasound multimodal score of endometrial in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:The endometrial ultrasonic multimodal score of recurrent spontaneous abortion patients is significantly lower than that of women of normal reproductive age.Therefore,the endometrial tolerance of recurrent spontaneous abortion patients can be evaluated by using the ultrasonic multimodal score,so as to avoid the influence of human factors on clinical diagnosis and treatment and ensure the objectivity of diagnosis and treatment results,which has significant clinical value.
