中药白芷防治骨质疏松性疼痛的机制探析

摘　　要：目的:探析白芷防治骨质疏松性疼痛(OPP)的中医理论机制。方法:基于中药药性理论及中医“不通则痛”致痛机理,结合“肠-骨轴”骨代谢调控模式,初步分析白芷的性状、性能及OPP的病因病机,并以此为基础阐释白芷防治OPP的作用机制。结果:OPP是由素体虚弱、后天调护失宜或外感风寒湿热之邪等所致阴阳气血失调、经脉运行不畅、骨络闭阻不通的一种全身慢性筋骨类疼痛。OPP主要病因包括禀赋与体质因素,年龄与性别因素,营养与运动因素;病机包括肾精亏虚,脾胃虚弱,血瘀气滞等。白芷可借自身药性功用如“辛温之性”散风寒湿、行气活血,“辛温芳香之性”和脾胃而发挥抗OPP的作用;白芷可通过靶向调控骨体相关经络等途径纠正机体阴阳气血不通引起的“不通”之病理状态,继而发挥抗OPP的作用;白芷亦可通过纠正“肠-骨轴”骨代谢调控模式紊乱发挥抗OPP作用。结论:“不通则痛”贯穿OPP发生发展过程的始终。白芷防治OPP具有确切的中医理论基础,为白芷防治OPP提供了中医临证依据。Objective:To analyze the theoretical mechanism of Chinese Herb Angelica dahurica preventing and treating osteoporotic pain(OPP).Methods:Based on Chinese herbal property theory and the pain mechanism of"stagnation leading to pain",combined with the"gut-bone axis"pattern of metabolism regulation,to analyze the characteristics and functional properties of Angelica dahurica,and the etiology and pathogenesis of OPP,and to explain the mechanism of Angelica dahurica preventing and treating osteoporotic pain.Results:OPP is a kind of chronic pain of muscles and bones in the whole body,which is caused by weakness of the body,acquired maladjustment or exogenous wind,cold,damp and heat evil.The main causes of OPP include endowment and physical factors,age and gender factors,nutrition and sports factors.Pathogenesis includes deficiency of kidney essence,weakness of spleen and stomach,blood stasis and qi stagnation.Angelica dahurica can play an anti OPP role by virtue of its own medicinal functions,such as"the nature of pungent warmth"dispersing wind and cold dampness,promoting qi and blood circulation,"the nature of pungent warmth and fragrance"harmonizing the spleen and stomach.Angelica dahurica can correct the"blocked"pathological state caused by the blockage of yin,yang,qi and blood through targeted regulation of bone related meridians,and then play the role of anti OPP.Angelica dahurica can also play the anti OPP role by correcting the disorder of"intestinal bone axis"bone metabolism regulation mode.Conclusion:"Stagnation leadingto pain"runs through the occurrence and development of osteoporotic pain.Angelica dahurica preventing and treating osteoporotic pain has exact traditional Chinese medicine theoretical basis,which provides traditional Chinese medicine clinical basis for Angelica dahurica preventing and treating OPP.
