FACE-Q量表评估自体耳软骨修复成年单侧唇裂继发鼻畸形的手术效果

摘　　要：目的采用FACE-Q量表评估自体耳软骨修复成年单侧唇裂继发鼻畸形的手术效果,为自体耳软骨修复的改良提供参考。方法选取2016年2月—2021年10月于徐州医科大学附属医院就诊的单侧唇裂继发鼻畸形成年患者共20例。所有患者均切取自体耳甲软骨、耳甲艇软骨,经测量裁剪后植入患侧鼻翼、鼻小柱,缝合固定,重塑鼻小柱及鼻翼外形,修复鼻畸形。术前、术后6个月拍摄下颌抬颏位照,对比手术前后鼻孔高度、鼻翼外角最低点到面中线的距离。采用19个FACE-Q系列量表评估患者的满意度。结果所有患者顺利完成手术。术前左右鼻孔高度、鼻翼外角最低点到面中线的距离差异明显(P<0.05),术后左右鼻孔高度、鼻翼外角最低点到面中线的距离无明显差异(P>0.05)。FACE-Q量表结果显示,手术效果得到患者的认可。其中“心理功能”选项的满意度最高,说明该手术可以显著改善患者心理功能。“鼻孔形状,鼻厚度”满意度较低。结论经FACE-Q系列量表证实,自体耳甲软骨修复成年唇裂继发鼻畸形具有较高的满意度。Objective To evaluate the effectiveness of autologous ear cartilage in repairing nasal deformity secondary to unilateral cleft lip in adults by FACE-Q scale,and to provide reference for proper modification.Methods A total of 20 patients with nasal deformity secondary to unilateral cleft lip who were admitted to the Affiliated Hospital of Xuzhou Medical University from February 2016 to October 2021 were enrolled.In the study,their autogenous concha cartilage and concha boat cartilage were collected followed by measurement and cutting,before being implanted into the affected nasal alar and columella.After suturing and fixing,the columella and alar were reshaped,and nasal deformity was repaired.The patients were photographed in the forward position and chin raising position before and six months after surgery.The height of the left and right nostrils and the distance from the lowest point of the outer corner of the nasal wing to the midline of the face were compared before and after surgery.Patient satisfaction was evaluated by FACE-Q scales.Results All the patients successfully finished the surgery.The height of the left and right nostrils and the distance from the lowest point of the outer corner of the nasal wing to the midline of the face were significantly different before surgery(P<0.05),but no statistical difference was found after surgery(P>0.05).According to FACE-Q scale results,all patients showed high satisfaction toward the surgery,where the best satisfaction was seen on psychological function,but the satisfaction toward nostril shape and nasal thickness was relatively low.Conclusions FACE-Q scale results indicate that autogenous concha cartilage shows good effect in repairing nasal deformity secondary to adult cleft lip.
