摘 要:目的:利用支气管镜联合快速现场评价(ROSE)技术分析儿童肺炎支原体肺炎的肺部病理。方法:选择60例2020年7月1日-2021年2月28日中山市妇幼保健院确诊的肺炎支原体肺炎患儿,在支气管镜下观察患儿肺部病变,并利用毛刷刷取支气管或肺部组织制成玻片,利用ROSE技术判读、分析其病变特点。结果:60例患儿均可见支气管内膜红肿、糜烂,50例见鹅卵石样增生,渗出物明显,5例出现塑型性支气管炎。60例患儿均可见纤毛柱状上皮细胞肿胀,部分可见化生为鳞状上皮细胞。大部分病例见炎症细胞浸润,包括:巨噬细胞、淋巴细胞、中性粒细胞,所有病例均可见巨噬细胞。结论:气管黏膜鹅卵石样增生,纤毛细胞肿胀、化生,巨噬细胞浸润等可作为肺炎支原体肺炎肺部病理改变的参考证据,支气管镜联合现场评价技术可为早期临床诊断提供依据。Objective:To analyze the pulmonary pathology of mycoplasma pneumoniae pneumonia in children by bronchoscopy and rapid on-site evaluation(ROSE)technology.Method:A total of 60 children who suffered from mycoplasma pneumoniae pneumonia were selected from Zhongshan Maternal and Child Health Hospital from July 1st,2020 to February 28th,2021,then observed the pulmonary pathology changes under bronchoscope,taken composite from lung and made slide sample,and analyzed the pathological characteristics through ROSE technology.Result:The endobronchial membranes of all 60 cases were red and swollen,erosive.Cobblestone-like hyperplasia and obviously exudates were found in 50 cases and plastic bronchitis change in 5 cases.Ciliated columnar epithelial cells swelling were seen in 60 cases,even metaplasia change of squamous epithelial cells in partly cases.Inflammatory cells included lymphocyte,neutrophils,macrophages infiltrated most cases,macrophages infiltrated all cases.Conclusion:Cobblestone-like hyperplasia of tracheal mucosa,swelling and metaplasia of ciliated cells and infiltration of macrophages can be used as reference for pulmonary pathological lesions from of mycoplasma pneumoniae pneumonia,bronchoscopic intervention combined with ROSE technology can provide early evidences for clinical diagnosis.
