针刺五心穴配合安神六穴治疗卒中后抑郁临床研究

摘　　要：目的:探讨针刺五心穴配合安神六穴治疗卒中后抑郁的临床疗效。方法:采用随机数表法,将医院收治的120例卒中后抑郁患者平均分为两组。对照组给予黛力新治疗,观察组在对照组治疗的基础上另给予针刺五心穴配合安神六穴治疗。统计中医症状评分、匹兹堡睡眠质量评分(PSQI)和汉密尔顿抑郁量表评分(HAMD),观察临床疗效、血浆5-羟色胺(5-HT)、去甲肾上腺素(NE)、25羟基-D_(3)[25(OH)D_(3)]水平及不良反应。结果:与治疗前相比,治疗后两组中医症状评分、PSQI评分和HAMD评分均降低,差异具有统计学意义(P<0.05),且观察组均低于对照组,差异具有统计学意义(P<0.05);观察组总有效率为93.33%(56/60),较对照组的80.00%(48/60)更高,差异具有统计学意义(P<0.05);治疗后两组血浆5-HT、25(OH)D_(3)水平均升高,差异具有统计学意义(P<0.05),且观察组更高,差异具有统计学意义(P<0.05);治疗后两组患者血浆NE水平均降低,差异具有统计学意义(P<0.05),且观察组更低,差异具有统计学意义(P<0.05);两组未见严重不良反应。结论:卒中后抑郁患者采用针刺五心穴配合安神六穴治疗,能够改善卒中和抑郁症状,疗效显著,同时能够升高血浆5-HT、25(OH)D_(3)水平,降低NE水平,安全可靠。Objective:To explore the clinical effect of needling five-heart points and six calming Shen points in treatment of post-stroke depression.Methods:120 patients with post-stroke depression were randomly divided into the control group and the observation group.The control group was treated with Deanixin;on which basis,the observation group was also treated with needling five-heart points and six calming Shen points.Scores of TCM symptoms,PSQI and HAMD,clinical efficacy,plasma levels of 5-HT,NE and 25(OH)D_(3),as well as adverse reactions were observed in the two groups.Results:The scores of TCM symptom,PSQI and HAMD were decreased after the treatment compared with those before the treatment in the two groups(P<0.05),and the decreases in the observation group were more significant than those in the control group(P<0.05).The total effective rate of the observation group was 93.33%(56/60),which was significantly higher than 80.00%(48/60)of the control group(P<0.05).The plasma levels of 5-HT and 25(OH)D_(3) were increased after the treatment compared with those before the treatment in the two groups(P<0.05),and the increases in the observation group were more significant(P<0.05);the plasma level of NE was decreased after the treatment compared with that before the treatment in the two groups(P<0.05),of which the observation group was lower than that in the control group(P<0.05).There were no serious adverse reactions in both groups.Conclusion:Needling five-heart points and six calming Shen points can improve stroke symptoms and insomnia in treatment of post-stroke depression,and the curative effect is significant.It can increase the plasma levels of 5-HT and 25(OH)D_(3),and reduce the level of NE,which is safe and reliable.
