老年糖尿病患者自我忽视现状及影响因素研究

摘　　要：目的探究老年糖尿病患者自我忽视现状及影响因素,为老年糖尿病患者的针对性干预提供参考。方法采用一般资料调查表、老年人自我忽视评估量表、简版老年抑郁量表、日常生活能力量表、糖尿病自我管理行为量表、领悟社会支持量表对216例老年糖尿病患者进行问卷调查。结果老年糖尿病患者自我忽视总分为16.00(8.00,27.75)分,抑郁总分为5.00(3.00,8.00)分,日常生活能力总分为16.00(14.00,22.00)分,糖尿病自我管理行为总分为28.00(21.00,36.75)分,领悟社会支持总分为(41.82±8.97)分。多元线性回归分析显示,性别、是否独居、有无配偶、月收入、抑郁、日常生活能力、糖尿病自我管理行为、领悟社会支持是老年糖尿病患者自我忽视的主要影响因素(P<0.01或P<0.05)。结论老年糖尿病患者自我忽视处于偏低水平,针对男性、独居、无配偶、收入水平低、抑郁水平高、日常生活能力低、糖尿病自我管理行为差、领悟社会支持低的老年糖尿病患者,医护人员应采取健康教育、引导患者子女、社区卫生人员支持、进行心理关怀、鼓励患者活动与锻炼、积极参与社交活动等措施,降低老年糖尿病患者自我忽视水平。Objective To explore the current situation and influencing factors of self-neglect in elderly diabetic patients,and to provide reference for targeted intervention in elderly diabetic patients.Methods A questionnaire survey was conducted among 216 elderly patients with diabetes mellitus by using general information questionnaire,Elder Self-Neglect Assessment(ESNA),Geriatric Depression Scale-15(GDS-15),Activities of Daily Living Scale(ADL),Summary of Diabetes Self-care Activities(SDSCA)and Perceived Social Support Scale(PSSS).Results The total score of self-neglect of elderly diabetic patients was 16.00(8.00,27.75),the total score of depression was 5.00(3.00,8.00),the total score of daily living ability was 16.00(14.00,22.00),the total score of diabetes self-management behavior was 28.00(21.00,36.75),and the total score of comprehension of social support was(41.82±8.97).Multiple linear regression analysis showed that gender,living style,with or without spouse,monthly income,depression,daily living ability,diabetes self-management behavior and perception of social support were the main influencing factors of self-neglect in elderly patients with diabetes(P<0.01 or P<0.05).Conclusion Elderly diabetic patients are at a high level of self-neglect.For elderly diabetic patients who are male,live alone,and have no spouse,with low income,high levels of depression,low ADL,poor self-management behaviors for diabetes,and low understanding of social support,medical staff should take measures such as health education,guidance of patients'children,community health personnel support,psychological care,encouragement of patients'activities and exercise,and active participation in social activities,to reduce the level of self-neglect of senior diabetic patients.
