《医验录》吴天士寒入血室案1则辨析

摘　　要：“寒入血室”,即妇女时逢月经来潮或月经将净或产后气血大伤之际,血海空虚,风寒外邪乘虚而入与正气相搏,或机体阳气虚衰、温煦气化功能减退,虚寒内生结于血室所致之证,首次由新安医家吴天士在《医验录》中记载。此文就吴氏“寒入血室”案1则从发病机制、诊断及处方用药方面做详细分析,并对案中所述症状进行类证鉴别,以期为现代临床诊断及治疗此类病证予以参考。“Cold entering the blood chamber”,that is a woman’s menstruation comes or her menstruation will be clean or her postpartum qi and blood will be seriously injured,the blood sea is empty,wind cold and external evils take advantage of the deficiency to fight with the healthy qi,or the syndrome caused by the deficiency and decline of yang qi,the decline of warm gasification function,and the deficiency and cold inside the blood chamber,is recorded by Wu Tianshi,a Xin’an doctor,in the medical experience record for the first time.This paper makes a detailed analysis on the case of Wu’s“cold entering the blood chamber”from the aspects of pathogenesis,diagnosis and prescription medication,and differentiates the symptoms described in the case,in order to provide reference for modern clinical diagnosis and treatment of such diseases and syndromes.
