大活络胶囊治疗类风湿性关节炎合并骨质疏松性脊柱陈旧性骨折腰痛的临床研究

摘　　要：目的探讨大活络胶囊治疗类风湿性关节炎(RA)合并骨质疏松性脊柱陈旧性骨折腰痛的临床效果。方法选择2019年1月—2020年6月衡阳市中医医院南院门急诊科收治的RA骨质疏松性脊柱陈旧性骨折腰痛患者60例,按照随机数字表法分为试验组(30例)与对照组(30例),对照组予以美洛昔康片治疗,试验组加用大活络胶囊治疗,比较2组的中医证候积分、血清学指标[血清骨钙素(BGP)水平、白介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)、类风湿因子(RF)]。结果试验组治疗后的中医证候单项积分均显著低于对照组(P<0.05);试验组治疗后的血清BGP水平显著高于对照组(P<0.05),血清IL-1、TNF-α、RF水平显著低于对照组(P<0.05)。结论大活络胶囊治疗可有效改善RA合并骨质疏松性脊柱陈旧性骨折腰痛患者的腰痛症状,减轻机体炎症状态,促进骨细胞钙化,控制RA病情进展。Objective To investigate the clinical effect of Dahuoluo capsule in the treatment of rheumatoid arthritis(RA)with delayed osteoporotic vertebral fracture low back pain.Methods From January 2019 to June 2020,60 patients with RA delayed osteoporotic vertebral fracture low back pain in South Hospital of Hengyang Hospital of Traditional Chinese Medicine were selected and randomly divided into experimental group(n=30)and control group(n=30).The control group was treated with meloxicam tablets,and the experimental group was treated with Dahuoluo capsule.The traditional Chinese medicine(TCM)syndrome scores and serological indexes[serum osteocalcin(BGP)level,interleukin-1(IL-1),tumor necrosis factor-α(TNF-α),rheumatoid factor(RF)]were compared between the two groups.Results After treatment,the single score of TCM syndrome in experimental group was significantly lower than that in control group(P<0.05).After treatment,the serum BGP level of experimental group was significantly higher than that of control group(P<0.05),and the serum IL-1,TNF-αand RF levels of experimental group were significantly lower than those of control group(P<0.05).Conclusion Dahuoluo capsule can effectively improve the symptoms of low back pain in RA patients with delayed osteoporotic vertebral fracture,reduce the inflammatory state,promote the calcification of bone cells,and control the progress of RA.
