褪黑素在子痫前期中的作用机制研究进展

摘　　要：褪黑素是一种脂溶性激素,最初发现是由松果体释放,具有昼夜节律的特点,在妊娠期间主要由胎盘产生,线粒体中合成的褪黑素水平较高。褪黑素在母体和胎儿中的作用是多方面的,在二者中均具有直接清除自由基和间接抗氧化活性的作用。研究表明,褪黑素与其受体的结合可以减少胎盘细胞死亡,促进胎盘发育,防止母体高血压的发生,改善新生儿预后。子痫前期是孕产妇发病和死亡的重要原因,目前,临床认为该病是由于胎盘血管受损,大量抗血管形成因子释放入血,导致母体血管内皮损伤和功能障碍。此外,胎盘功能障碍和氧化应激均参与了子痫前期的发病过程。最近的研究表明在成功妊娠过程中,褪黑素可作为活性氧(ROS)和氮类的高效清除剂,通过刺激内源性抗氧化酶在胎盘中发挥重要作用,合理应用褪黑素可以安全地延长子痫前期孕妇的孕周,缓解相应的高血压症状。本文对褪黑素在子痫前期中的作用机制研究进展进行综述,以期能为有效预防子痫前期提供帮助。Melatonin is a lipid-soluble hormone originally found to be released by the pineal gland and has the characteristics of circadian rhythm.Melatonin is mainly produced by the placenta during pregnancy,and it is synthesised in mitochondria at high levels.The role of melatonin is multifaceted with direct free radical scavenging and indirect antioxidant activities in both the mother and the fetus.Studies showed that the binding of melatonin to its receptors can reduce placental cell death,promote placental development,prevent the development of maternal hypertension and improve newborn prognosis.Preeclampsia is an important cause of maternal morbidity and mortality.At present,it is clinically believed that the disease is due to the damage of placental blood vessels and the release of a large number of anti angiogenic factors into the blood,resulting in maternal vascular endothelial damage and dysfunction.In addition,both placental dysfunction and oxidative stress are involved in the pathogenesis of preeclampsia.Recent studies have shown that melatonin can act as an efficient scavenger of reactive oxygen species(ROS)and nitrogen during successful pregnancy,and play an important role in placenta by stimulating endogenous antioxidant enzymes.Reasonable use of melatonin can safely prolong the gestational weeks of pregnant women with preeclampsia,alleviate the corresponding hypertension symptoms.This paper reviews the research progress of the mechanism of melatonin in preeclampsia,in order to provide help for the effective prevention of preeclampsia.
