头部震动法在水平半规管BPPV治疗中的临床疗效分析

摘　　要：目的 探讨头部震动法在水平半规管的良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)治疗中的疗效。方法 选择2019年1月—2020年12月于北京怀柔医院神经内科门诊就诊的380例水平半规管BPPV患者,随机分为两组,观察组采用头部震动法,共190例;对照组采用Barbecue法,共190例;比较两组患者的疗效、前庭症状指数(vestibular symptom index,VSI)、眩晕残障评定量表(dizziness handicap inventory,DHI)和复发情况上的差异。结果 观察组1次治疗有效率为94.74%、2次治疗有效率98.42%、3次有效率99.47%,均高于对照组的73.68%、86.84%、94.74%,两组差异有统计学意义(P <0.05);两组患者的VSI评分及DHI评分均较治疗前有显著改善,差异有统计学意义(P <0.05),但观察组下降的幅度更为显著,差异有统计学意义(P <0.05);观察组和对照组治疗后1月内复发率分别为13.16%和15.79%,观察组略好于对照组,但差异无统计学意义(P> 0.05)。结论 头部震动法治疗水平半规管BPPV的疗效优于Barbecue法,适合临床推广应用。Objective To investigate the clinical effects of head vibration method in treatment of horizontal semicircular canal bengin paroxysmal positional vertigo(BPPV).Methods A total of 380 patients with horizontal semicircular canal who were treated in our hospital from January 2019 to December 2020 were enrolled in the study.They were stochastically divided into two groups.One group with 190 cases,was observation group,who were treated in head vibration maneuver.The other was control group,with 190 cases,who were treated with Barbecue’s repositioning maneuver.The clinical efficacy,vestibular symptom index(VSI),dizziness handicap inventory(DHI)and relapse rate were observed and compared among the two groups.Results The total effective rate in observation group with once treatment was 94.74%,with 2 times treatment was 98.42%,with 3times treatment was 99.47%,which was dramatically higher than that(73.68%,86.84%,94.74%) in control group,and the differences were statistically significant(P 0.05).Conclusion The clinical effects of head vibration maneuver in treatment of horizontal semicircular canal BPPV are superior to those by Barbecue’s repositioning maneuver,it is suitable for clinical application.
