肾衰饮对糖尿病肾病大鼠足细胞标志蛋白、足细胞裂孔膜蛋白影响

摘　　要：目的研究肾衰饮对糖尿病肾病(DKD)大鼠足细胞标志蛋白(PCX)、足细胞裂孔膜蛋白(Nephrin)的影响。方法选取SPF级雄性SD大鼠72只随机分为空白组、DKD组、厄贝沙坦组、肾衰饮低剂量组、肾衰饮中剂量组、肾衰饮高剂量组。采用高脂高糖饲料喂养联合链脲佐菌素腹腔注射建立DKD大鼠模型。厄贝沙坦组采用厄贝沙坦2.7 mg/mL灌胃;肾衰饮低、中、高剂量组分别采用肾衰饮2.79、5.58、11.16 g/mL灌胃;空白组、DKD组采用等体积生理盐水灌胃。连续给药8周后检测各组大鼠尿蛋白(PRO)、总胆固醇(TC)、血肌酐(Scr)、甘油三酯(TG)、血尿素氮(BUN)、高密度脂蛋白胆固醇(HDL-C)、葡萄糖(GLU)、谷丙转氨酶(ALT)、低密度脂蛋白胆固醇(LDL-C)、谷草转氨酶(AST)含量;苏木素-伊红(HE)染色观察肾脏组织病理形态学变化;酶联免疫吸附试验检测血清碱性纤维母细胞生长因子(bFGF)、胰岛素样生长因子(IGF)、单核细胞趋化蛋白-1(MCP-1)表达水平;蛋白免疫印迹法检测肾脏组织PCX、Nephrin蛋白表达水平。结果与空白组比较,DKD组大鼠GLU、ALT、AST、PRO、Scr、BUN、TC、TG、LDL-C水平显著升高,HDL-C水平显著下降(P<0.01);肾小球发生硬化性改变、上皮细胞颗粒样变性、囊腔明显变窄,肾小管周围大量炎性细胞浸润;血清bFGF、IGF、MCP-1表达水平显著升高(P<0.01);PCX、Nephrin水平显著降低(P<0.01)。与DKD组比较,厄贝沙坦组和肾衰饮各剂量组大鼠GLU、ALT、AST、PRO、Scr、BUN、TC、TG、LDL-C水平显著降低,HDL-C水平显著上升(P<0.05,P<0.01);肾脏病理损害不同程度减轻;厄贝沙坦组、肾衰饮高剂量组大鼠血清bFGF、IGF、MCP-1表达水平均有不同程度下降(P<0.01);厄贝沙坦组、肾衰饮高剂量组大鼠PCX、Nephrin水平均有不同程度上升(P<0.05,P<0.01)。结论肾衰饮能够有效降低DKD大鼠血糖、血脂、肝功能、肾功能、蛋白尿水平,抑制血清bFGF、IGF、MCP-1表达,Objective To investigate the effect of Shenshuai Yin(肾衰饮,SSY)on podocyte marker protein(PCX)and podocyte pore membrane protein(Nephrin)in diabetic kidney disease(DKD)rats.Methods 72 SPF male SD rats were randomly separated into blank group,DKD group,irbesartan group,SSY low dose group,SSY medium dose group and SSY high dose group.DKD rat model was modeling by high-fat and high sugar diet and intraperitoneal injection of streptozotocin.The irbesartan group was given irbesartan 2.7 mg/mL.The SSY low-dose group,medium dose group and high-dose group were given Shenshuai Yin 2.79,5.58 and 11.16 g/mL.Blank group and DKD group were given equal volume physiological saline.After 8 weeks of continuous administration,urinary protein(PRO),total cholesterol(TC),serum creatinine(Scr),triglyceride(TG),blood urea nitrogen(BUN),high density lipoprotein cholesterol(HDL-C),glucose(GLU),alanine aminotransferase(ALT),low density lipoprotein cholesterol(LDL-C)and aspartate aminotransferase(AST)were measured.Hematoxylin eosin(HE)staining was used to observe the pathological changes of kidney.The expression levels of basic fibroblast growth factor(bFGF),insulin-like growth factor(IGF)and monocyte chemoattractant protein-1(MCP-1)in serum were detected by enzyme-linked immunosorbent assay.The expression levels of PCX and Nephrin protein in renal tissue were detected by Western blotting.Results Compared with blank group,the levels of GLU,ALT,AST,PRO,Scr,BUN,TC,TG and LDL-C in DKD group increased significantly,and the level of HDL-C decreased significantly(P<0.01);sclerosing changes,granular degeneration of epithelial cells,obvious narrowing of capsule cavity and infiltration of a large number of inflammatory cells around renal tubules occurred in glomerulus;the expression levels of bFGF,IGF and MCP-1 in serum increased significantly(P<0.01);the levels of PCX and Nephrin decreased significantly(P<0.01).Compared with DKD group,the levels of GLU,ALT,AST,PRO,Scr,BUN,TC,TG and LDL-C in irbesartan group and SSY each dose group decreased si
