循经针灸联合加味独活寄生汤治疗腰椎间盘突出症效果研究

摘　　要：目的探讨循经针灸联合加味独活寄生汤治疗腰椎间盘突出症临床效果。方法选取2018年2月—2021年2月淮南东方医院集团总院收治的腰椎间盘突出症患者66例,按照随机数字表法分成观察组(n=33)与对照组(n=33),对照组采用加味独活寄生汤治疗,观察组采用循经针灸联合加味独活寄生汤治疗,比较两组视觉模拟评分(VAS评分)、腰椎日本骨科协会评估治疗分数(JOA评分)、健康状况调查简表(SF-36)评分、临床症状评分及炎性因子水平。结果两组治疗后腰椎JOA评分、SF-36评分较治疗前有明显提高(P<0.05),VAS评分明显降低(P<0.05);且与对照组相比,观察组腰椎JOA评分、SF-36评分升高(P<0.05),VAS评分降低(P<0.05)。观察组治疗后步态、直腿抬高试验、感觉障碍、运动障碍、日常活动影响评分明显高于对照组(P<0.05)。两组治疗后白介素-6(IL-6)水平较治疗前均明显降低(P<0.05);且观察组水平低于对照组(P<0.05)。结论循经针灸联合加味独活寄生汤应用于腰椎间盘突出症治疗,可有效减轻腰椎间盘突出患者的疼痛感,降低患者的炎性反应,增强腰椎功能,提高患者生活质量。Objective To explore the clinical effect of meridian acupuncture combined with Modified Duhuo Jisheng Decoction(独活寄生汤)in the treatment of lumbar disc herniation.Methods 66 patients with lumbar disc herniation treated in Huainan oriental hospital group from February 2018 to February 2021 were randomly divided into observation group(n=33)and control group(n=33).The control group was treated with Modified Duhuo Jisheng Decoction,and the observation group was treated with meridian acupuncture combined with Modified Duhuo Jisheng Decoction.The VAS score,lumbar JOA score,SF-36 score,clinical syndrome score and inflammatory factor levels were compared.Results There was no significant difference in the scores of VAS,lumbar JOA and SF-36 between the two groups before treatment(P>0.05).After treatment,the scores of lumbar JOA and SF-36 were significantly higher than those before treatment(P<0.05),and the VAS score was significantly lower(P<0.05).Compared with the control group,the scores of lumbar JOA and SF-36 in the observation group were higher(P<0.05),and the VAS score was lower(P<0.05).The gait,straight leg elevation test,sensory disturbance,motor disturbance and daily activity influence scores in the observation group were significantly higher than those in the control group(P<0.05).After treatment,the level of interleukin-6(IL-6)of two groups were lower than before treatment.The level of IL-6 in the observation group was lower than that in the control group(P<0.05).Conclusion The application of meridian acupuncture combined with Modified Duhuo Jisheng Decoction in the treatment of lumbar disc herniation can effectively reduce the pain of patients with lumbar disc herniation,reduce the inflammatory reaction of patients,enhance lumbar function and improve the quality of life of patients.
