左炔诺孕酮宫内节育系统与炔诺酮片治疗围绝经期异常子宫出血患者的效果比较

摘　　要：目的比较左炔诺孕酮宫内节育系统(曼月乐)与炔诺酮片治疗围绝经期异常子宫出血患者的效果。方法选取60例围绝经期异常子宫出血患者为研究对象,根据随机数字表法分为对照组与研究组,每组30例。对照组给予炔诺酮片治疗,研究组给予曼月乐治疗。观察两组治疗效果、止血时间、不良反应发生率、血红蛋白水平及子宫内膜厚度。结果研究组治疗总有效率93.33%,高于对照组的73.33%,差异均有统计学意义(P<0.05)。研究组止血时间为(1.83±1.02)d,对照组为(2.42±0.65)d,研究组止血时间短于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血红蛋白水平及子宫内膜厚度比较差异无统计学意义(P>0.05);治疗后,研究组血红蛋白水平高于对照组,子宫内膜厚度少于对照组,差异均有统计学意义.(P<0.05)。结论采用曼月乐治疗围绝经期异常子宫出血患者,治疗效果较好,有助于提升患者止血效果,缩短止血时间,预防不良反应的出现,临床治疗效果显著,值得临床推广应用。Objective To compare the effect of levonorgestrel intrauterine birth control system(Mirena)and norethisterone tablets in the treatment of perimenopausal patients with abnormal uterine bleeding.Methods A total of 60 patients with abnormal uterine bleeding during perimenopause were selected as the research object and divided into the control group and the study group according to the random number table method,30 cases in each group.The control group was treated with norethinone tablets,and the observation group was treated with mirena.The treatment effect,hemostatic time,incidence of adverse reactions,hemoglobin level and endometrial thickness of the two groups were observed.Results The total effective rate of the study group was 93.33%,higher than that of the control group(73.33%),and the differences were statistically significant(P<0.05).The hemostatic time of the study group was(1.83±1.02)d,and that of the control group was(2.42±0.65)d,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in hemoglobin level and endometrial thickness between the two groups(P>0.05).After treatment,the hemoglobin level of the study group was higher than that of the control group,and the endometrial thickness was lower than that of the control group,and the diferences were statistically signifcant(P<0.05).Conclusion Mirena in the treatment of perimenopausal patients with abnormal uterine bleeding,the treatment effect is good,help to improve the hemostatic effect of patients,shorten the hemostatic time,prevent the occurrence of adverse reactions,clinical treatment effect is significant,worthy of clinical application.
