摘  要:目的 探讨妊娠高血压患者孕晚期血清PLGF、VEGF与胎儿生长受限的关系。方法 选取2020年7月至2021年8月武汉市中医医院收治的产检、确诊并分娩为胎儿生长受限的45例妊娠高血压孕妇作为研究对象(FGR组),另选取同期胎儿正常的50例妊娠高血压孕妇作为正常组,对两组患者的一般资料和生化指标等进行单因素分析,重点比较两组患者孕晚期血清PLGF、VEGF表达水平与胎儿生长受限的相关性,Logistic多元回归分析影响妊娠高血压患者孕晚期胎儿生长受限的危险因素。结果 两组患者临床资料显示FGR组孕周、产前BMI、吸烟、羊水过少、分娩方式、白蛋白、纤维蛋白原、新生儿体质量、尿酸、APTT、TT、子宫动脉PI、RI、S/D差异有统计学意义(P<0.05);FGR组母血、脐血中PLGF、VEGF表达水平显著低于正常组(P<0.05);Logistic多因素回归分析显示吸烟、产前BMI、羊水过少、尿酸、S/D值、孕晚期血清PLGF及VEGF是妊娠高血压患者发生胎儿生长受限的关键因素;Spearman相关分析结果显示,妊娠高血压患者母血、脐血PLGF、VEGF的表达水平与胎儿生长受限呈正相关;ROC曲线分析表明妊娠高血压患者孕晚期血清PLGF、VEGF表达水平对胎儿生长受限的预测能力较强。结论 孕晚期血清PLGF、VEGF表达水平下降提示与胎儿生长受限有关,其可作为预测妊娠高血压患者胎儿生长受限的可靠指标,在改善患者妊娠结局方面具有重要作用。Objective To investigate the relationship between serum PLGF, VEGF and fetal growth restriction in patients with gestational hypertension. Methods 45 pregnant women with gestational hypertension who were admitted to Wuhan Hospital of Traditional Chinese Medicine from July 2020 to August 2021 and diagnosed as fetal growth restriction were selected as the research objects(FGR group), and 50 pregnant women with normal fetus in the same period were selected as the normal group. Univariate analysis was used to analyze the general data and laboratory test indexes of the two groups, focus on the correlation analysis between the expression of serum PLGF and VEGF in the third trimester of pregnancy and fetal growth restriction. Multivariate logistic regression analysis was used to analyze the risk factors of fetal growth restriction in the third trimester of pregnancy in patients with gestational hypertension. The ROC curve judges the prediction efficiency of the model.Results The clinical data of the two groups showed that there were significant differences in gestational weeks, prenatal BMI,smoking, oligohydramnios, mode of delivery, albumin, fibrinogen, uric acid, neonatal weight, APTT, TT and uterine artery S/D in FGR group(P<0.05). The expression levels of PLGF and VEGF in maternal blood and umbilical cord blood in FGR group were significantly lower than those in normal group(P<0.05). Logistic multiple regression analysis showed prenatal BMI,smoking, oligohydramnios, uric acid, PI, RI, S/S/D ratio, serum PLGF and VEGF in the third trimester of pregnancy were the key factors of fetal growth restriction in patients with gestational hypertension. Spearman correlation analysis showed that the expression levels of PLGF and VEGF in maternal blood and umbilical cord blood of patients with gestational hypertension were positively correlated with fetal growth restriction.
