摘　　要：目的通过Meta分析法系统评价亚甲基四氢叶酸还原酶(MTHFR)C677T基因多态性与中国人群妊娠糖尿病的相关性。方法计算机检索中国知网、万方数据库、维普信息资源系统等中文文献数据库,收集关于MTHFR C677T基因多态性与妊娠糖尿病相关的病例对照研究,提取所需数据并用RevMan5.3统计软件采用随机效应模型进行Meta分析,利用Stata 12.0软件检验发表偏倚。结果共纳入10个病例-对照研究,合计病例5938例,对照13813例;Meta分析结果显示:MTHFR基因C677T多态性的T比C、TT比CC、TC比CC、(TT+TC)比CC、TT比(TC+CC)5种基因遗传模型总效应均与妊娠糖尿病发生风险相关(P<0.05);在亚组分析中,南北不同地区分布、样本量是否大于200例的MTHFR基因C677T多态性5种基因模型均与妊娠糖尿病发生有一定相关性(P<0.05)。结论MTHFR C677T基因多态性与中国人群妊娠糖尿病易感性密切相关,存在明显地域差异性,且C-T突变可能是妊娠糖尿病发生的一个重要遗传风险因素。Objective To systematically evaluate the correlation between C677T of methylene tetrahydrofolate reductase(MTHFR)gene polymorphism and gestational diabetes in Chinese population by Meta-analysis.Methods Computer search Chinese literature database,square database,wei pu information resource system,collect about MTHFR C677T gene polymorphism and gestational diabetes related case-control studies,extract the required data and RevMan 5.3 statistical software using Meta analysis,using Stata 12.0 software to test publication bias.Results A total of 10 case-control studies were included,in total of 5938 cases,there were 13813 control cases.Meta-analysis showed that the total effects of the five genetic models[T/C,TT/CC,TC/CC,(TT+TC)/CC,TT/(TC+CC)]of the T C677T polymorphism of MTHFR gene were associated with the risk of gestational diabetes(P<0.05).In subgroup analysis,the five gene models of MTHFR gene C677T polymorphism distributed in different regions in the north and south,and whether the sample size was greater than 200 cases were all related to the occurrence of gestational diabetes(P<0.05).Conclusion MTHFR C677T gene polymorphism is closely related to gestational diabetes susceptibility in Chinese population,and C-T mutation may be an important genetic risk factor for gestational diabetes.
