摘 要:目的基于疾病诊断相关分组评价河北省11个地级市43所三级综合医院骨科疾病的医疗质量。方法采用国际MS-DRGs方法评价2017年7月1日-12月31日河北省11个地级市以及涉及到的43所三级综合医院骨科疾病DRGs总量、病例组合指数、时间消耗指数、费用消耗指数等医疗质量情况并进行优劣排序,采用象限图对其医疗服务能力和医疗服务效率进行分析。结果研究的11个地级市平均DRGs总量为2948.14,其中最高医院为3542.34,最低地级市DRGs总量为851.16,最低医院为7.70;平均CMI为0.88,CMI最高的地级市为0.98,最高医院为1.05,最低地级市为0.82,最低医院为0.7;平均TCI为0.87,最高TCI地级市为1.30,最高医院为1.52,最低TCI地级市为0.62,最低医院为0.54;平均CCI为0.75,最高CCI地级市为1.17,最高医院为1.90,最低CCI地级市为0.49,最低医院为0.34。四象限图分析,医疗服务能力最强的地级市为City I(10,0.92),最强的医院为City H2(9,1.00),医疗服务能力最弱的地级市为City F(9,0.82),最弱的医院为City A5(3,0.77);医疗服务效率最强的地级市有9个,较强的医院有22所。结论河北省各个地级市三级综合医院医疗服务质量较好,但存在医院间医疗服务能力、医疗服务效率参差不齐,需要引起足够重视。Objectives This study aims to evaluate the medical quality of orthopedic diseases in 43 tertiary general hospitals in 11 prefecture-level cities in Hebei province based on diagnosis-related group.Methods This study used the international MS-DRGs method to evaluate the medical quality conditions,such as the total number of DRGs of orthopedic diseases,combination index of medical records,time consumption index and expense consumption index,in 43 tertiary general hospitals in 11 prefecture-level cities in Hebei province and ranked them based on their quality.Quadrant Chart Analysis was performed on the medical services capabilities and medical services efficiency.Results The average number of DRGs among 11 investigated prefecture-level cities was 2948.14.The highest amount of DRGs in a hospital was 3542.34.The lowest amount of total DRGs in a prefecture-level city was 851.16.The lowest amount of DRGs in a hospital was 7.70.The average CMI was 0.88,with the highest CMI in a prefecture-level city of 0.98,the highest CMI in a hospital of 1.05,the lowest CMI in a prefecture-level city of 0.82 and the lowest CMI in a hospital of 0.7.The average TCI was 0.87,with the highest TCI in a prefecture-level city of 1.30,the highest TCI in a hospital of 1.52,the lowest TCI in a prefecture-level city of 0.62 and the lowest TCI in a hospital of 0.54.The average CCI was 0.75,with the highest CCI in a prefecture-level city of 1.17,the highest CCI in a hospital of 1.90,the lowest CCI in a prefecture-level city of 0.49 and the lowest CCI in a hospital of 0.34.According to the result of Quadrant Chart Analysis,the prefecture-level city with the best medical services capability was City I(10,0.92),the hospital with the best medical services capability was City H2(9,1.00).The prefecture-level city with the poorest medical services capability was City F(9,0.82),the hospital with the poorest medical services capability was City A5(3,0.77).There were 9 prefecture-level cities with the best medical service efficiency and 22 hospitals with goo
