某三甲医院骨科脊柱融合术住院费用的影响因素研究

摘　　要：目的通过了解骨科脊柱融合术的住院费用的影响因素,分析针对不同因素的干预措施,为控制费用提供线索。方法从某三甲医院病案首页系统中抽取2018年1月1日-2020年12月31日骨科脊柱融合术患者,共1512例,对住院费用相关的影响因素进行多元回归分析。结果平均住院费用为122014.69±43257.70元。有术后并发症的平均住院费用可达159714.18元。经过多元线性回归分析,F=124.261,P<0.05,影响因素包括住院天数、年龄、其他诊断个数、是否合并脑血管病等。结论住院天数超过1周、年龄≥60岁老年住院患者及术后合并症多(≥5个)是医院骨科脊柱融合术住院费用的影响因素。将住院日控制在1周内、针对高龄女性合并症多的脊柱融合术的患者提高诊疗效果、避免手术合并症和并发症的发生有利于减少住院费用。Objectives This study aims to investigate the influencing factors of the hospitalization expense of spinal fusion surgery and analyze the intervention methods corresponding to different factors,in order to provide evidence for managing the expenses.Methods A total of 1512 cases of patients underwent spinal fusion surgeries in the Orthopedics Department from January 1,2018 to December 312020 were selected from the medical record front page system.This study used multiple regression analysis to investigate the expense related influencing factors.Results The average hospitalization expense of the investigated patients was 122014.69±43257.70 Yuan.The expenses of patients with postoperative complications could reach 159714.18 Yuan.According to multiple regression analysis,the F value was 124.261,P<0.05.The influencing factors included the length of stay,age,the number of other diagnoses,whether cerebrovascular disease was combined,etc.Conclusions The main factors influencing the hospitalization expenses of spinal fusion surgery in the Orthopedics Department in the investigate hospital are over a week of hospitalization,age no less than 60 and multiple postoperative complications(≥5).The hospitalization expense can be reduced by controlling the length of stay within one week and improve the treatment effect spinal fusion surgery towards elderly female patient with multiple complications in order to prevent the co-occurrence of both surgical comorbidity and complications.
