基于数据挖掘探讨刘霞教授治疗小儿免疫性血小板减少症的用药规律

摘　　要：目的 本研究通过数据挖掘方法探讨刘霞教授治疗小儿免疫性血小板减少症的用药规律,进一步总结刘霞教授的学术经验。方法 收集2019年12月—2020年11月在刘霞教授门诊就诊的免疫性血小板减少症患者的医案处方,借助“中医传承计算平台V3.0”软件,进行组方规律分析。结果 系统提取190个医案处方,涉及药物98种,医案处方药物的四气以平、寒、温为主,平性药物数量最多;五味分布中以甘味最多,其次为苦味;归经分布中以肝、肾、肺为主。功效分布中补虚类最多,其次为清热类。以聚类分析的方法提取的核心组合处方3个,均为刘霞教授临床经验方的主要组成部分。结论 刘霞教授治疗小儿免疫性血小板减少症以清消补三法为主,注重药物配伍,标本兼治,以治虚为本,治热为标,根本在于调补脾肾,治疗经验中蕴含“调肝”的思想。Objective To explore the medication rule of Professor Liu Xia in the treatment of children’s immune thrombocytopenia by data mining, and further summarize her academic experience. Methods Medical prescriptions of children with immune thrombocytopenia who visited Professor Liu Xia’s outpatient clinic from December 2019 to November 2020 were collected. With the help of Traditional Chinese Medicine Inheritance calculation platform V3.0 software, the law of its prescriptions was analyzed. Results 190 medical prescriptions were extracted systematically, involving 98 kinds of drugs. The four qi of medical prescriptions were mainly composed of flat, cold and warm. Among the five flavors, sweet was the most, followed by bitter. In the distribution of meridian tropism, liver, kidney and lung were the most common. In efficacy distribution, tonifying deficiency category was the most, followed by clearing heat category. Three core combination prescriptions were extracted by cluster analysis, which were the main components of Professor Liu Xia’s clinical experience prescriptions. Conclusion Professor Liu Xia’s treatment of immune thrombocytopenia in children is based on three Methods purification dispersion and tonicity, heat clearing and blood cooling, and pays attention to drug compatibility. On the whole, to treat deficiency as foundation, to treat heat as the standard, to treat both the symptoms and root causes, the fundamental is to regulate and tonify the spleen and kidney, and the thought of “regulating liver” is contained in the treatment experience.
