摘 要:目的探讨高效液相色谱(HPLC)法和酶放大免疫(EMIT)法测定人血清伏立康唑(VRC)质量浓度的相关性。方法收集医院2019年至2020年住院和门诊患者常规VRC检测样本2045份,对其中59份样本同时采用两种检测方法,并采用双侧配对t检验分析结果的相关性;建立回归方程并绘制Bland-Altman图;按质量浓度结果分为A组(<1.0µg/mL)、B组(1.0~5.5µg/mL)和C组(>5.5µg/mL),采用Wilcoxon检验、Passing-Bablok线性回归和Bland Altman法分析3组的相关性及一致性。分析2045份样本血药浓度测定结果。结果两种测定方法具有高度相关性(r=0.879,P<0.05);不同质量浓度下,两种测定方法一致性很差,EMIT法检测结果均高于HPLC法。结论临床测定VRC血药浓度时,应关注不同检测方法的差异,相应调整临床用药剂量;HPLC法和EMIT法检测结果不具有直接比较意义。Objective To investigate the correlation between the determination results of voriconazole(VRC)in human serum by the high-performance liquid chromatography(HPLC)method and enzyme amplified immunoassay technique(EMIT)method.Methods A total of 2045 samples of routine VRC testing were collected from inpatients and outpatients in the hospital from 2019 to 2020.Bilateral paired t-test was used to analyze the correlation between the results of 59 samples determined by the two test methods.The regression equation was established and the Bland-Altman plot was drawn.The experiment was divided into group A(<1.0µg/mL),group B(1.0-5.5µg/mL)and group C(>5.5µg/mL)according to the results of mass concentration,and the Wilcoxon test,Passing-Bablok linear regression and Bland-Altman method were used to analyze the correlation and consistency of the two methods in the three groups.The determined results of serum drug concentration of 2045 samples were analyzed.Results The results determined by the two methods had a high correlation(r=0.879,P<0.05).At different mass concentrations,the consistency of the two methods was very poor,and the result determined by EMIT method was higher than that determined by the HPLC method.Conclusion When determining the VRC serum concentration,we should pay attention to the difference of different determination methods and adjust the clinical dosage according to the results.The results determined by the HPLC method and EMIT method could not be compared directly.
