摘　　要：目的促进临床合理使用注射用两性霉素B。方法从医院住院系统中提取2019年至2020年使用该药的住院病历,利用Excel软件随机抽取100份。根据相关原则、指南及文献,选取26个评价指标并分为一级指标(9个)和二级指标(17个),结合药品说明书评价用药合理性,并分别赋分;构建属性层次模型,采用加权逼近理想解排序法计算各病例与最优方案的接近度(Ci),分析用药合理性。结果100份病历的Ci多介于0.5~0.8,其中0.4~<0.5,0.5~<0.6,0.6~<0.7,0.7~<0.8,0.8~<0.9,0.9~1.0的分别有1,15,50,30,4,0例(占比分别为1.00%,15.00%,50.00%,30.00%,4.00%,0)。结论该院两性霉素B临床不合理用药现象较明显,医院应加强其使用管理。Objective To promote the rational use of Amphotericin B for Injection in the clinic.Methods The inpatient medical records of Amphotericin B for Injection from 2019 to 2020 were extracted from the hospital inpatient system,and 100 records were randomly selected through Excel software.According to relevant principles,guidelines and literature,twenty-six evaluation indicators were selected and divided into first-level indicators(nine indicators)and second-level indicators(seventeen indicators).The rationality of drug use was evaluated based on the drug package insert and scored separately.The attribute hierarchical model was constructed,and the weighted technique for order preference by similarity to an ideal solution(TOPSIS)method was used to calculate the closeness(C_(i))between each case and the optimal scheme,and to analyze the rationality of drug use.Results The C of 100 medical records mostly ranged from 0.5 to 0.8,of which 0.4≤C_(i)<0.5 accounted for 1.00%(one case),0.5≤C_(i)<0.6 accounted for 15.00%(15 cases),0.6≤C_(i)<0.7 accounted for 50.00%(50 cases),0.7≤C_(i)<0.8 accounted for 30.00%(30 cases),0.8≤C_(i)<0.9 accounted for 4.00%(four cases),while the C_(i) without records ranged from 0.9 to 1.0.Conclusion The irrational phenomenon of the clinical use of Amphotericin B for Injection in the hospital is obvious.The hospital should strengthen the management of the clinical use of Amphotericin B for Injection.
