土鳖虫的临床应用及其用量探究

摘　　要：通过搜集古医籍及现代医家临床经验,总结出土鳖虫具有以下特点:1)汤剂用量为4~23.75 g,常用剂量为6~10 g,丸散剂用量为0.1~16.75 g。2)根据疾病、证型、症状寻求最佳用量,如治疗类风湿关节炎、强直性脊柱炎、硬皮病、慢性胃炎、慢性肾衰、急性腰扭伤等疾病,可用土鳖虫活血通络止痛,剂量为6~23.75 g,丸散剂量为0.1~16.75 g;各种肿瘤、慢性肝病、慢性萎缩性胃炎、乳腺增生、前列腺增生等疾病,可用土鳖虫活血散瘀、消癥破坚,剂量为4~12 g,丸散剂量为0.1~3.76 g;治疗心脑血管病及其后遗症、偏头痛等疾病,可用土鳖虫活血化瘀、通利血脉,剂量为6~12 g;治疗痛风时常取其利湿化浊之功,剂量为6~10 g。3)根据疾病、证型及症状配伍相应中药,如活血通络止痛配伍露蜂房、当归、鸡血藤、片姜黄、炮山甲、独一味、山茱萸等;活血散瘀、消癥破坚配伍炙鳖甲、水蛭、地龙、昆布、海藻等;活血化瘀、通利血脉配伍川芎、桃仁、蜈蚣、人参;利湿化浊配伍土茯苓、萆薢、威灵仙。Based on the collection of ancient medical books and clinical experience of modern physicians,the following characteristics of ground beetles are summed up:(1)Its dosage range in decoction is 4-23.75 g,and its commonly used dosage range is 6-10 g,while in pills or powders 0.1-16.75 g.(2)Its optimal dosage should be determined according to different diseases,syndrome types and symptoms.For example,when used for promoting blood circulation to remove obstructions in collaterals and relieving pains in the treatment of rheumatoid arthritis,ankylosing spondylitis,scleroderma,chronic gastritis,chronic renal failure,acute lumbar sprain and other diseases,its dosage range is 6-23.75 g,while in pills or powders 0.1-16.75 g.For promoting blood circulation and dissipating blood stasis,relieving symptoms and resolving masses in the treatment of various tumors,chronic liver diseases,chronic atrophic gastritis,hyperplasia of mammary glands,hyperplasia of prostate and other diseases,its dosage range is 4-12 g,while in pills or powders 0.1-3.76 g.For activating blood circulation to dissipate blood stasis and dredging blood vessels in the treatment of cardio-cerebrovascular diseases and their sequelae,migraine and other diseases,its dosage range is 6-12 g.For promoting diuresis and removing turbidity in the treatment of gout,its dosage range is 6-10 g.(3)Ground beetles are compatible with corresponding Chinese medicines according to various diseases,syndrome types and symptoms.For example,for promoting blood circulation to remove obstructions in collaterals and relieving pains,it is often combined with Nidus vespae,Chinese angelica,suberect spatholobus stems,sliced turmeric,Squama manis,Lamiophlomis rotata,asiatic cornelian cherry fruits,etc.For promoting blood circulation and dissipating blood stasis,relieving symptoms and resolving masses,it is often combined with fried turtle carapace,leeches,earthworms,kelps,seaweeds,etc.For activating blood circulation to dissipate blood stasis and dredging blood vessels,it is often combined
