耳针、穴位贴敷联合导乐仪对产妇产褥期阴道微生态的影响

摘　　要：目的探讨耳针、穴位贴敷联合导乐仪对产妇产褥期阴道微生态的影响。方法选择126例产褥期阴道微生态失衡患者,随机化分为对照组与观察组,各63例。对照组采用常规治疗,观察组在对照组基础上加用耳针、穴位贴敷联合导乐仪治疗。比较2组治疗后阴道微生态平衡恢复率、阴道微生态平衡评价指标、阴道灌洗液中炎症因子肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)、阴道黏膜免疫因子CD3^(+)、CD4^(+)/CD8^(+)、NK细胞水平;随访6个月,统计2组阴道感染类疾病发生率。结果观察组治疗后阴道微生态平衡恢复率、阴道菌群密度Ⅱ、Ⅲ级、阴道菌群多样性Ⅱ、Ⅲ级、革兰阳性杆菌优势菌、阴道清洁度Ⅰ度患者占比高于对照组,阴道pH>4.5患者占比低于对照组(P<0.05);观察组治疗后TNF-α、IL-8低于对照组(P<0.05);观察组治疗后CD3^(+)、CD4^(+)/CD8^(+)、NK细胞高于对照组(P<0.05);观察组阴道感染类疾病发生率(3.17%)低于对照组(15.87%)(P<0.05)。结论耳针、穴位贴敷联合导乐仪应用于产褥期阴道微生态失衡患者,可改善炎症反应,促进阴道微生态平衡恢复,提高阴道黏膜免疫功能,降低阴道感染类疾病发生率。Objective To investigate the effect of auricular acupuncture and acupoint applications combined with the Doula instrument on the vaginal microecology during puerperium.Methods A total of 126 patients with vaginal microecological imbalance during puerperium from March 2019 to January 2022 were enrolled and randomly divided into a control group and an observation group,with 63 patients in each group.The control group was treated with a routine therapy,while the observation group was treated with ear acupuncture,acupoint applications and the Doula instrument on the basis of the control group.The recovery rate and evaluation indexes of vaginal microecological balance,the levels of inflammatory factors,tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),vaginal mucosal immune factors CD3^(+),CD4^(+)/CD8^(+),and NK cells were compared between the two groups after treatment.The two groups were followed up for half a year and their incidence of vaginal infections was counted.Results After treatment,the recovery rate of vaginal microecological balance,the percentage of patients with vaginal flora densityⅡandⅢ,vaginal flora diversityⅡandⅢ,Gram positive dominant bacteria and vaginal cleanliness I in the observation group were higher than those in the control group,and the proportion of patients with vagina pH>4.5 was lower than that in the control group(P<0.05).The levels of TNF-αand IL-8 in the observation group were lower than those in the control group(P<0.05),while the levels of CD3^(+),CD4^(+)/CD8^(+)and NK cells were higher than those in the control group(P<0.05),and the incidence of vaginal infections in the observation group(3.17%)was lower than that in the control group(15.87%)(P<0.05).Conclusion In the treatment of puerperal patients with vaginal microecological imbalance,ear acupuncture and acupoint applications combined with the Doula instrument can improve the inflammatory response,promote the recovery of vaginal microecological balance,improve the immune function of vaginal mucosa,and reduce the inci
