腮腺浅叶良性肿瘤次全切术治疗腮腺浅叶良性肿瘤的临床疗效

摘　　要：目的 探讨腮腺浅叶良性肿瘤行次全切术的临床疗效。方法 52例腮腺浅叶良性肿瘤患者随机分为两组,对照组使用腮腺浅叶全切除术,观察组使用腮腺浅叶良性肿瘤次全切术,对比手术指标、并发症情况、面神经损伤、生活质量。结果观察组手术时间、切口长度、术中出血量、解剖面神经分支数、住院时间均少于对照组(P<0.05);观察组皮下积液或涎瘘、面部凹陷畸形、暂时性面瘫、耳颞神经综合征、耳垂麻木等并发症发生率明显低于对照组(P<0.05);观察组面神经1度、2度、3度、4度以及总发生率均低于对照组(P<0.05);观察组术后8个维度的SF-36评分均高于对照组(P<0.05)。结论 腮腺浅叶良性肿瘤行次全切术的临床疗效显著,最大限度保留了腮腺组织,避免面神经损伤,降低并发症几率,改善生活质量。Objective To investigate the clinical effect of subtotal resection of benign tumors in the superficial lobe of parotid gland.Methods 52 patients with benign tumors of superficial parotid gland treated were randomly divided into two groups.The control group was treated with total parotidectomy and the observation group was treated with subtotal parotidectomy.The operation indexes,complications,facial nerve injury and quality of life were compared.Results The operation time,incision length,intraoperative bleeding,anatomical branches of facial nerve and hospital stay in the observation group were less than those in the control group(P<0.05);The incidence of complications such as subcutaneous effusion or salivary fistula,facial depression deformity,temporary facial paralysis,ototemporal nerve syndrome and earlobe numbness in the observation group was significantly lower than that in the control group(P<0.05);The degree 1,degree 2,degree 3 and degree 4 of facial nerve and the total incidence in the observation group were lower than those in the control group(P<0.05);The SF-36 scores of 8 dimensions in the observation group were higher than those in the control group(P<0.05).Conclusion Subtotal resection of benign tumors in the superficial lobe of parotid gland has significant clinical effect,which can preserve the parotid gland tissue to the greatest extent,avoid facial nerve injury,reduce the probability of complications and improve the quality of life.
