药品质控问题为导向的医疗机构IOS管控模式构建与实施

摘　　要：目的探索构建质控问题为导向的药品管控模式,确保医院药品质量安全,提升用药安全,体现药师价值。方法从理念(ideology,I)、系统(operation,O)和人员(staff,S)(即IOS)三个维度,通过管理机制、技术手段和人才培养三大创新,采取四大举措即“建立药品三级质量安全监管体系、打造药品质量管控责任共同体、创建智能AI全景药品质控系统、建立创新型质控药师实战能力提升培训体系”进行药品质量管控。结果通过近三年实践,该管控模式解决了药品质控中存在的“管理机制落后,不能齐抓共管各担其责;监管技术缺失,信息无法引领动态监管;质控能力不足,质量管理成效低”三大难题,各药房核心质控指标明显改善。与实施前相比,实施后药师对药品质控知识掌握度大幅提升;处方调配差错量显著降低;药品破损率逐年下降;药品盘点误差率大幅降低;药品账物相符率稳步提升并趋于100%。结论本研究构建了药品质控问题为导向的医疗机构IOS管控模式,通过管理机制创新、技术手段创新和人才培养创新,实现了医疗机构药品质量全流程、精细化管控,充分发挥了药师的质控作用,努力实现药品质量安全零差错的目标。Objective To construct drug management mode oriented by quality control,to ensure the drug quality in hospitals,and to improve medication safety and reflect the value of pharmacists.Methods From three dimensions of ideology,operation and staff(IOS),four major measures were taken to carry out the management and quality control of drugs through three innovations including management mechanism,technical means and personnel training.Then,a three-level supervision system,a community of responsibility,and an artificial intelligence system for drug quality and safety control,and an innovative training system for improving the practical ability of quality control pharmacists would be established.Results Through nearly three years′practice,this management mode has solved three major problems in the management and quality control of drugs.The first problem is that management mechanism is too backward to shoulder all parties′responsibilities.The second problem is the lack of regulatory technology,leading to no dynamic supervision.The third one is quality control ability is insufficient and the quality management effectiveness is low.Therefore,the core quality control indexes of the pharmacy are significantly improved.Besides,pharmacists′knowledge on drug quality control has been greatly improved,and the number of prescriptions dispensing errors are reduced significantly.The drug breakage rate is decreased year by year,and the error rate of drug inventory has greatly dropped.Lastly,the consistency rate of drug accounts and goods has increased steadily,tending to 100%.Conclusion IOS management mode oriented by quality control of drugs in medical institutions is constructed in this study.The innovation of management mechanism,technical means and talent training are adopted to realize the fine control of drug quality in the whole process of drug circulation in hospitals.The role of pharmacists′quality control has been brought into full play,which helps to achieve the goal of zero error in the drug quality and safety.
