摘　　要：目的:以前列腺癌根治术(RP)后病理结果为标准,回顾性分析前列腺癌(PCa) MRI皮下脂肪厚度(SFT)及前列腺周围脂肪厚度(PPFT)与Gleason评分之间的关系。方法:纳入RP术前行MRI检查的患者144例,年龄(69.5±6.7)岁,分别在矢状位及横断位T2WI上测量SFT和PPFT,用Spearman相关系数(ρ)分析SFT、PPFT、年龄、前列腺癌特异性抗原(PSA)与Gleason评分相关性,用logistic回归分析SFT、PPFT与不同级别(Gleason评分≤7和Gleason评分>7)前列腺癌之间的相关性。结果:SFT和前列腺周脂肪最小厚度(PPFTm)分别为(29.34±10.1)mm和(4.89±2.5)mm。Gleason评分与术前PSA显著正相关(ρ=0.410,P=0.000),与SFT显著负相关(ρ=0.225,P=0.007),与PPFTm呈弱相关(ρ=0.182,P=0.029)。多因素回归分析显示,PSA、SFT和PPFTm是高级别PCa的独立预测因子。结论:以RP术后病理结果为参考标准,MRI图像SFT、PPFTm与Gleason评分呈轻、中度显著相关,是高级别PCa的独立预测因素。Purpose:To retrospectively evaluate the relationship between subcutaneous fat thickness(SFT)and periprostatic fat thickness(PPFT)on MRI and Gleason score of prostate cancer(PCa)using pathological results after radical prostatectomy(RP)as the reference standard.Methods:One hundred and forty-four patients aged(69.5±6.7)years who underwent MRI examination before RP were included.SFT and PPFT were measured on sagittal and transverse T2WI,respectively.Spearman correlation coefficient(ρ)was used to analyze the correlation between SFT,PPFT,age,prostate cancer specific antigen(PSA)and Gleason score.Logistic regression was used to analyze the correlation between SFT,PPFT and different grades of prostate cancer(Gleason score≤7 and Gleason score>7).Results:SFT and PPFTm were(29.34±10.1)mm and(4.89±2.5)mm,respectively.Gleason score was positively correlated with preoperative PSA(ρ=0.410,P=0.000),negatively correlated with SFT(ρ=0.225,P=0.007),and weakly correlated with PPFTm(ρ=0.182,P=0.029).Multivariate regression analysis showed that PSA,SFT and PPFTm were independent predictors of high grade PCa.Conclusions:SFT and PPFTm on MRI have showed a mild to modest but significant correlation with Gleason score of PCa with pathological results after RP as the reference standard,and are independent predictive factors for high-grade PCa.
