摘　　要：目的:提高对婴儿中胚叶肾瘤(CMN)的影像学表现认识。方法:选择手术完整切除、术后病理资料证实的12例患儿CMN,其中男性9例,女性3例;年龄28周胎龄~1岁,平均年龄2.4个月。行超声、CT、MRI检查,分析其影像学表现,观察肿块病变的影像学特征,记录肿块的位置、大小、强化方式、包膜、血管或血管外的侵犯。结果:6例患儿行腹部超声+CT检查,1例仅行MRI检查,4例仅行CT检查,1例为CT+MRI。病理诊断结果为中胚叶肾瘤,均表现为左/右肾区占位,体积均较大、边界部分清晰。超声表现为混合回声占位。CT平扫为等低密度伴不同程度的坏死、其中2例伴钙化,增强后肿瘤不均匀强化、中央条索样强化明显。肿瘤MRI表现为T1WI低等信号、T2WI等高信号,增强后明显强化,坏死区无明显强化,均无周围结构浸润。病理结果为经典型CMN7例、细胞型CMN3例,混合型CMN2例。结论:超声联合CT和/或MRI对CMN诊断具有一定的临床价值。Purpose:To provide better understanding of imaging characteristics of congenital mesoblastic nephroma(CMN).Methods:Twelve cases with CMN confirmed by post-operative pathology results were enrolled,including 9 males and 3 females.The patients were with age ranged from 28 weeks of gestational age to 1 year old with an average age of 2.4 months old.Their images of ultrasound,CT and MRI were analyzed,including the imaging characteristics of the mass:location,size,contrast enhancement,capsule,vascular or extravascular invading.Results:Both abdominal ultrasound and CT were performed in 6 cases,MRI only in 1 case,CT in 4 cases,and both CT and MRI in 1 case.The mass located in left or right renal.Imaging characteristics of CMN showed mixed echo on ultrasound,iso-low density with different degrees of necrosis on CT,calcification in 2 cases,and heterogeneous enhancement after contrast.MRI findings were iso-intense signal on T1-weighted image,iso-high intense signal on T2-weightd image,with heterogeneous enhancement,but the necrosis area was not enhanced.The pathology results of CMN showed that classical type was in 7 cases,cellular type in 3 cases,and mixed type in 2 cases.Conclusion:Ultrasound combined with CT and/or MRI has significant clinical application value for CMN diagnosis.
