摘  要:目的 探讨《外科用药指导手册》在临床用药指导中的作用。方法 随机选取2020年1月至2021年12月在上海市第六人民医院普通外科接受住院治疗的144例患者作为研究对象,依照入院先后顺序将其分为2组,其中2020年1-12月入院的患者为对照组,2021年1-12月入院的患者为观察组。对照组患者进行常规护理,观察组患者在对照组基础上,由经过《外科用药指导手册》培训的低年资外科护士为其护理。比较2组患者入院时和出院时用药知识掌握程度、用药行为及满意度。结果 144例患者中,4例患者因病情加重、抗拒护理干预或中途转院被剔除,最终对照组和观察组各70例。入院时,2组患者用药知识测试题得分比较,差异无统计学意义(P>0.05)。出院时,2组患者用药知识测试题得分均较入院时显著提高(P<0.01),且观察组高于对照组(P<0.01)。入院时,2组患者用药安全行为量表(assessment of inpatients involvement in patient medication safety scale,AIIPMSS)各领域评分及总分比较,差异均无统计学意义(P均>0.05)。出院时,2组患者用药安全行为评分均较入院时显著提高(P均<0.01),且观察组均高于对照组(P均<0.05)。入院时,2组患者对护理工作满意度量表(scale of inpatient satisfaction,SIS)中的各因子评分及总分比较,差异均无统计学意义(P均>0.05)。出院时,2组患者对SIS中的健康教育及指导、个人特质与服务态度、出院指导评分及SIS总分均较入院时显著提高(P均<0.01),且观察组均高于对照组(P均<0.01)。结论 开展《外科用药指导手册》的相关培训,可提高临床用药指导效率与质量,提升患者用药知识掌握程度,改善患者用药行为,提高患者满意度,对临床用药具有重要意义。Objective To explore the role of "Surgical Medication Instruction Manual" in clinical medication guidance.Methods 144 patients who were hospitalized for treatment in the General Surgery Department of the Sixth People’s Hospital from January 2020 to December 2021 were randomly selected as research subjects.They were divided into two groups in accordance with the sequence of admission.The patients admitted from January to December,2020 were designated as the control group,while those admitted from January to December,2021 were designated as the observation group.The patients in the control group just received routine nursing,while the patients in the observation group were nursed by junior surgical nurses who had been trained in the "Surgical Medication Instruction Manual".The mastery of medication knowledge,drug use behavior and satisfaction rate both at admission and discharge were observed and compared between the patients of the 2 groups.Results Of the 144 patients,4 patients were excluded from study due to aggravation of disease,resistance to nursing intervention or intermediate transfer,and there were 70 patients in both groups.There were no significant differences in the scores of medication knowledge tests at admission,when comoparisons were made between the two groups(P>0.05).At discharge,the scores of medication knowledge tests in both groups were significantly raised as compared with those at admission,with the scores of the observation group being higher than those of the control group(P<0.01).At admission,there were no significant differences in various scores and total scores in the Assessment of "Inpatients Involvement in Patient Medication Safety Scale"(AIIPMSS),when comparisons were made between the two groups(all P>0.05).At discharge,the scores in the patients of the 2 groups significantly increased,as compared with those at admission(all P<0.01),with the scores of the AIIPMSS,as well as various and total scores being all higher than those in the control group(all P<0.05).At admission,there were
