血清IL-31、CCL27及外周血Treg细胞水平在慢性荨麻疹患者中的检测意义

摘　　要：目的:分析血清白细胞介素-31(IL-31)、人皮肤T细胞虏获趋化因子(CCL27)及外周血调节性T细胞(Treg)水平在慢性荨麻疹患者中的检测意义。方法:回顾性分析我院收治的199例荨麻疹患者的临床资料,根据病程、治疗时间及复发情况将其分为急性组(急性荨麻疹,n=86)和慢性组(慢性荨麻疹,n=113),比较两组患者血清IL-31、CCL27及外周血Treg细胞水平差异,采用Pearson分析荨麻疹病程与血清IL-31、CCL27及外周血Treg细胞水平的相关性。采用受试者工作特征(ROC)曲线,评估血清IL-31、CCL27及外周血Treg细胞水平对早期鉴别诊断急性荨麻疹与慢性荨麻疹的效能。结果:慢性组血清IL-31、CCL27水平显著高于急性组(P<0.05),外周血Treg细胞水平显著低于急性组(P<0.05)。患者荨麻疹病程时间与血清IL-31、CCL27水平呈正相关(P<0.05),与外周血Treg细胞水平呈负相关(P<005)。IL-31、CCL27、Treg曲线AUC明显大于参考线(P<0.05),其cut-of值为1522 ng/L、1125.704 pg/m、7.454%。结论:血清IL-31、CCL27、外周血Treg细胞与慢性荨麻疹病程关系紧密,IL-31、CCL27水平越高或Treg细胞水平越低,说明悲者免疫功能越异常,而且IL-31、CCL27、外周血Treg细胞水平还能有助于有效诊断急、慢性荨麻疹,具有较高临床检测价值。Objective:To analyze the detection significance of serum interleukin-31(IL-31),human skin T cell capture chemokine(CCL27)and peripheral blood regulatory T cells(Treg)levels in patients with chronic urticaria.Methods:The clinical data of 199 patients with urticaria admitted to our hospital were retrospectively analyzed.According to the course of disease,treatment time and recurrence,they were divided into acute group(acute urticaria,n=86)and chronic group(chronic urticaria,n=113).To compare the difierences in serum IL-31,CCL27 and peripheral blood Treg cells levels between the two groups of patients,Pearson was used to analyze the correlation between the course of urticaria and the levels of serum IL-31,CCL27 and peripheral blood Treg cells.The receiver operating characteristic(R.OC)curve was used to evaluate the efficacy of serum IL-31,CCL27 and peripheral blood Treg cells levels in the early difierential diagnosis of acute urticaria and chronic urticaria.Results:The levels of serum IL-31 and CCL27 in the chronic group were significantly higher than those in the acute group(P<0.05),and the levels ofTreg cells in the peripheral blood were significantly lower than those in the acute group(PV0.05).The duration of urticaria in patients was positively correlated with serum IL-31 and CCL27 levels(P<0.05),and negatively correlated with peripheral blood Treg cell levels(P<0.05).The AUC of IL-31,CCL27 and Treg curves were significantly larger than the reference line(P<0.05),and their cut-off values were 1.522 ng/L,1125.704 pg/ml,and 7.454%.Conclusion:Serum IL-31,CCL27,and peripheral blood Treg cells are closely related to the course of chronic urticaria.The higher the level of IL-31,CCL27 or the lower the level ofTreg cells,the more abnormal the patientzs immune functiort Moreover,the levels of the IL-31,CCL27,and peripheral blood Treg cells,which can contribute to the e&ctive diagnosis of acute and chronic urticaria,have high clinical detection values.
