2017年至2019年某院非计划再入院与非计划再次手术医疗质量指标评价分析

摘　　要：目的分析某院非计划再入院及非计划再次手术发生情况,探讨降低非计划再入院与非计划再次手术发生率的措施及管理方法。方法对某院2017年1月1日-2019年12月31日出院患者非计划再入院例数3804例、非计划再次手术59例病例资料进行回顾性分析。结果非计划再入院发生率总体呈上升趋势,三年发生率差异有统计学意义(P<0.05),非计划再入院病种主要为支气管肺炎、妊娠合并子宫瘢痕、高危妊娠监督、早产不伴分娩、子宫内粘连、先兆流产、宫颈恶性肿瘤、肺炎;非计划再次手术三年发生率差异无统计学意义(P>0.05),儿外科非计划再次手术例数占比达60%,消化系统手术占比较大,手术并发症是导致非计划再次手术的主要原因。结论医院应加强再入院管理、围手术期管理,有效降低非计划再入院率、再手术率,提高医疗质量,保障妇儿安全。Objectives To analyze the incidence of unplanned readmission and unplanned reoperation in a hospital,and explore the measures and management methods to reduce the incidence of unplanned readmission and unplanned reoperation.Methods The data of 3804 cases of unplanned readmission and 59 cases of unplanned reoperation in a hospital from January 1st,2017 to December 31st,2019 were analyzed retrospectively.Results The overall incidence of unplanned readmission showed an upward trend,and there was a significant difference in the three-year incidence(P<0.05).The main diseases of unplanned readmission were bronchopneumonia,pregnancy complicated with uterine scar,high-risk pregnancy supervision,premature delivery without delivery,intrauterine adhesion,threatened abortion,cervical malignancy and pneumonia;There was no significant difference in the three-year incidence of unplanned reoperation(P>0.05).The number of unplanned reoperations in pediatric surgery accounted for 60%,and gastrointestinal and other digestive system operations accounted for a large proportion.Surgical complications were the main reason for unplanned reoperation.Conclusions Hospitals should strengthen readmission management and perioperative management,effectively reduce the rate of unplanned readmission and reoperation,improve medical quality and ensure the safety of women and children.
