摘　　要：目的探讨普通胃镜检查对患者心肺血管及生理反应的影响。方法收集2021年9月至2021年12月在首都医科大学附属北京安贞医院消化内科行普通胃镜检查的214例患者的临床资料,按年龄分为青年组(<45岁)116例、中年组(45~59岁)48例和老年组(>59岁)50例,记录胃镜检查开始前、检查中及检查结束时三组患者的心率、血氧饱和度的变化以及烦躁、焦虑、干呕等生理反应的发生情况。结果胃镜检查前、检查中和检查结束时,患者的心率分别为(79.8±13.6)次/min、(102.3±19.7)次/min、(87.9±16.8)次/min,不同时点两两比较差异均有显著性(P<0.05);血氧饱和度分别为(97.8±1.5)%、(97.6±1.5)%、(97.9±1.5)%,检查前、检查结束时分别与检查中比较差异有显著性(P<0.05)。青年组患者各时点的心率均高于中年组和老年组,老年组患者各时点的血氧饱和度均低于青年组和中年组。青年组患者出现烦躁、焦虑的比例为19.8%,明显高于中年组的6.3%和老年组的8.0%(P<0.05);青年组患者出现干呕的比例为27.6%,明显高于中年组的14.6%和老年组的16.0%(P<0.05)。结论普通胃镜检查明显影响患者的心率和血氧饱和度,青年患者胃镜检查期间生理反应明显多于中老年患者。普通胃镜检查过程中应监测患者的心肺血管情况及生理反应,必要时给予相应的护理措施,如吸氧和护理安慰。Objective To investigate the effect of ordinary gastroscopy on cardiopulmonary vessels and physiological response.Method The 214 patients undergoing general gastroscopy in Beijing Anzhen Hospital,Affiliated to Capital Medical University,from September 2021 to December 2021 were selected.The 214 patients were divided into youth group(<45 years old,116 cases),middle-aged group(45-59 years old,48 cases)and elderly group(>59 years old,50 cases).The changes in heart rate,blood oxygen saturation and the physiological reactions of irritability,anxiety,retching were recorded.Result Before,during and after gastroscopy,214 patient’s heart rate were(79.8±13.6)beat/min,(102.3±19.7)beat/min,(87.9±16.8)beat/min,differences were significant(P<0.05).Before,during and after gastroscopy,214 patient’s oxygen saturation were(97.8±1.5)%,(97.6±1.5)%,(97.9±1.5)%,there were significant differences in blood oxygen saturation between those obtained before examination and before examination and during examination(P<0.05).The heart rate before,during and after examination was higher than in the middle-aged and elderly group;the oxygen saturation before,during and after gastroscopy was lower than in the youth and middle-age group.The proportion of irritability and anxiety in the youth group was 19.8%,significantly higher than 6.3%(middle-age group)and 8.0%(elderly group)(P<0.05);that in the youth group was 27.6%,significantly higher than 14.6%in middle-age group and 16.0%in elderly group(P<0.05).Conclusion General gastroscopy obviously affects patient heart rate and blood oxygen saturation,and young patients have more physiological reactions during gastroscopy than middle-aged and elderly patients.In the process of general gastroscopy,cardiopulmonary vascular conditions and physiological reactions should be monitored,and give corresponding nursing measures,such as oxygen inhalation and nursing comfort,when necessary.
