摘　　要：目的:研究精益六西格玛(LSS)管理方法在降低手术室共用内窥镜设备故障率中的应用价值。方法:运用LSS管理方法中的界定、测量、分析、改进和控制5个阶段流程对内窥镜设备故障进行分析,采用组织系统模型(SIPOC)流程管理及鱼骨图等方法,分析影响内窥镜设备故障率的关键环节,根据分析发现的重点关注内容,提出建立内窥镜设备信息化管理系统、规范操作定期培训和监督机制以及定期保养和质量控制管理机制的改进方案,对比2019-2021年LSS管理方法实施前后手术室共用内窥镜设备故障数据变化情况。结果:内窥镜设备信息化管理系统正式运行后的2021年度与运行前的2019年度相比,内窥镜手术量增长11.82%,而设备故障下降12.82%,维修费用降低24.50%。结论:LSS管理方法能够形成内窥镜设备的闭环管理,明确各环节责任主体,为内窥镜设备监督管理提供客观依据,降低手术室共用内窥镜设备故障率。Objective:To study the application value of Lean Six Sigma(LSS)management method in reducing the failure rate of shared endoscope equipment in operating room.Methods:The key links affecting failure rate of endoscope equipment were analyzed by using the he LSS 5-stage process including define,measure,analyze,improve and control,SIPOC(Suppliers,Inputs,Process,Outputs and Customers)process management model and fishbone diagrams,etc..Based on key concerns found in the analysis,an improvement program for the establishment of an information management system for endoscope equipment,a mechanism for regular training and supervision on standardized operation,and a mechanism for regular maintenance and quality control management were proposed.The changes in the failure data of shared endoscope equipment in the operating room before and after the implementation of the LSS management method were compared.Results:Compared with the year of 2021 after the official launch of the information management system for endoscopic equipment with the year 2019 before its launch,the number of endoscopic surgeries increased by 11.82%,meanwhile the number of endoscopic equipment failures decreased by 12.82%,and the maintenance cost decreased by 24.50%.Conclusion:LSS management method can form the closedloop management of endoscope equipment,define the responsible party of each link,provide an objective basis for the supervision and management of endoscope equipment,and reduce the failure rate of shared endoscope equipment in the operating room.
