大理白族自治州围孕期女性血清中同型半胱氨酸水平及其意义

摘　　要：目的了解大理白族自治州围孕期女性血清中同型半胱氨酸(Homocysteine,Hcy)水平及临床指导意义。方法选择2019年12月—2020年5月于大理白族自治州妇幼保健院进行围孕期保健的健康女性750例,将所有受检者按照年龄不同分为≤24岁组115例,25~29岁组304例,30~34岁组256例,≥35岁组75例,按照孕期不同分为早孕期组(0~13^(+6)周)213例,中孕期组(14~27^(+6)周)218例,晚孕期组(28周以后)290例以及产后组29例。使用胱硫醚循环酶速率法检测血清中Hcy水平,使用荧光定量PCR法检测口腔黏膜脱落细胞中5,10-亚甲基四氢叶酸还原酶(MTHFR)基因C677T位点多态性。结果≤24岁组、25~29岁组、30~34岁组、≥35岁组血清中Hcy水平比较差异有显著性(F=6.57,P<0.05)。早孕期组、中孕期组、晚孕期组、产后组血清中Hcy水平比较差异有显著性(F=16.21,P<0.05)。受检者CC、CT、TT基因型血清中Hcy水平比较差异有显著性(F=3.04,P<0.05)。结论围孕期女性血清中Hcy水平与年龄、孕周以及MTHFR基因C677T位点多态性显著相关。高龄和MTHFR基因C677T位点中TT基因型围孕期女性血清中Hcy水平的监测,对保障母婴健康具有重要的临床意义。Objective To investigate the serum level of homocysteine(Hcy)and its clinical significance in periconceptional women in Dali Bai Autonomous Prefecture of China.Methods A total of 750 periconceptional women who attended Maternal and Child Health Hospital of Dali Bai Autonomous Prefecture from December 2019 to May 2020 were enrolled,and accor-ding to their age,they were divided into≤24 years group with 115 women,25-29 years group with 304 women,30-34 years group with 256 women,and≥35 years group with 75 women;according to their stage of pregnancy,they were divided into early pregnancy group(0-13^(+6) weeks)with 213 women,middle pregnancy group(14-27^(+6) weeks)with 218 women,late pregnancy group(≥28 weeks)with 290 women,and postpartum group with 29 women.Enzymatic cycling assay for cystathionine was used to measure the serum concentration of Hcy,and quantitative real-time PCR was used to detect 5,10-methylenetetrahydrofolate reductase(MTHFR)gene C677T polymorphism.Results There was a significant difference in serum Hcy level between the≤24 years,25-29 years,30-34 years,and≥35 years groups(F=6.57,P<0.05),and there was also a significant difference in serum Hcy level between the early pregnancy group,the middle pregnancy group,the late pregnancy group,and the postpartum group(F=16.21,P<0.05).There was a significant difference in serum Hcy level between the periconceptional women with CC,CT,or TT genotype(F=3.04,P<0.05).Conclusion Serum Hcy level in periconceptional women is associated with age,gestational weeks,and MTHFR gene C677T polymorphism.The monitoring of serum Hcy level should be strengthened for the women with an old age and MTHFR C677T TT genotype,which has important guiding significance for periconceptional healthcare.
