摘 要:目的探讨Bcl-Ⅰ基因多态性对哮喘患儿糖皮质激素治疗后复发的影响。方法选取2018年4月—2020年12月收治的小儿哮喘80例设为观察组,另外选取同期体检健康的儿童80例设为对照组。观察组入院后行糖皮质激素治疗。2组均行Bcl-Ⅰ基因多态性检测。观察组治疗结束后行3个月随访,依据患儿是否复发分为未复发组59例与复发组21例。收集观察组临床资料,对比观察组与对照组Bcl-Ⅰ基因多态性检测结果,复发组与未复发组Bcl-Ⅰ基因多态性的基因型检出率差异。影响复发的因素采用多因素Logistic回归分析。结果观察组Bcl-Ⅰ基因多态性为GG基因型检出率高于对照组,CC基因型检出率低于对照组(P<0.01)。年龄≤6岁、过敏原暴露、合并鼻窦炎、合并过敏性鼻炎、哮喘严重程度为重度、Bcl-Ⅰ基因多态性为GG是哮喘患儿糖皮质激素治疗后复发的危险因素(P<0.05,P<0.01)。结论哮喘患儿Bcl-Ⅰ基因多态性为GG基因型检出率高,且GG基因型为糖皮质激素治疗后复发的危险因素。Objective To study the effect of Bcl-I gene polymorphism on the recurrence of asthma in children after glucocorticoid therapy.Methods Eighty children with asthma admitted from April 2018 to December 2020 were selected as the observation group,and another 80 healthy children undergoing physical examination during the same period were selected as the control group.The children in the observation group were treated with glucocorticoids after admission.Both groups were tested for Bcl-Ⅰgene polymorphism after admission(before treatment in the treatment group).Then,the children in the treatment group were followed up for 3 months after treatment.Children were divided into non-recurrence group(n=59)and recurrence group(n=21)according to the presence or absence of recurrence.The clinical data of the observation group were collected.The differences in results of Bcl-Ⅰgene polymorphism detection between the observation group and the control group as well as and the basic data information and the differences in genotypic detection rate of Bcl-Ⅰgene polymorphism between the recurrence group and the non-recurrence group were compared.Multivariate logistic regression analysis was used to analyze the factors influencing recurrence.Results The GG genotypic detection rate of Bcl-Ⅰgene polymorphism in the observation group was significantly higher than that in the control group,while the CC genotypic detection rate was lower than that in the control group(P<0.01).Age≤6 years,allergen exposure,concomitant sinusitis,concomitant allergic rhinitis,severe asthma,and GG genotype of Bcl-Ⅰgene polymorphism were risk factors for recurrence of asthma in children after glucocorticoid therapy(P<0.05,P<0.01).Conclusion The GG genotypic detection rate of Bcl-Ⅰgene polymorphism in asthmatic children is high,and GG genotype is a risk factor for recurrence after glucocorticoid therapy.
