摘 要:目的探讨血清同型半胱氨酸(Hcy)、β_(2)-微球蛋白(β_(2)-MG)及尿微量白蛋白(mAlb)对急性肾损伤的预测价值。方法选取2018年10月—2020年10月收治的急性肾损伤95例作为研究组,根据急性肾损伤分期标准将其另分为1期组34例、2期组30例和3期组31例。另选取同期接受健康体检的健康体检者96例作为对照组。比较研究组与对照组及不同急性肾损伤分期3组血清Hcy、β_(2)-MG和mAlb水平,以及不同急性肾损伤分期3组血清Hcy、β_(2)-MG和mAlb单独及联合检测阳性率;绘制受试者工作特征曲线(ROC)分析血清Hcy、β_(2)-MG和mAlb对急性肾损伤的预测价值。结果血清Hcy、β_(2)-MG和mAlb水平,研究组高于对照组,1期组低于2期组和3期组,2期组低于3期组(P<0.05,P<0.01)。不同急性肾损伤分期3组血清Hcy、β_(2)-MG和mAlb联合检测阳性率均明显高于单独检测阳性率(P<0.01)。ROC曲线分析结果显示,血清Hcy、β_(2)-MG和mAlb三者联合检测较单独检测对急性肾损伤预测价值的曲线下面积和敏感度、特异度高。结论血清Hcy、β_(2)-MG和mAlb水平与急性肾损伤患者发病和病情程度密切相关,三者联合检测对急性肾损伤的预测价值较高。Objective To study the predictive value of homocysteine(Hcy),β_(2)-microglobulin(β_(2)-MG)and microalbum inuria(mAlb)in acute kidney injury(AKI).Methods A total of 95 patients with AKI admitted to our hospital from October 2018 to October 2020 were collected as the research group.According to the staging criteria of AKI,they were subdivided into stage 1 subgroup(n=34),stage 2 subgroup(n=30),and stage 3 subgroup(n=31).In addition,96 healthy people who underwent physical examination in our hospital during the same period were selected as the control group.The differences in the levels of Hcy,β_(2)-MG,and mAlb between the research group and control group and among three subgroups according to different AKI staging were compared.The positive detection rates of Hcy,β_(2)-MG,and mAlb alone and in combination were compared among three subgroups according to different AKI staging.The receiver operating characteristic(ROC)curve was drawn to analyze the value of Hcy,β_(2)-MG,and mAlb in predicting AKI.Results The levels of Hcy,β_(2)-MG,and mAlb were higher in the research group than in the control group,lower in stage 1 subgroup than in stage 2 subgroup and stage 3 subgroup,and,and lower in stage 2 subgroup than in stage 3 subgroup(P<0.05,P<0.01).The positive detection rates of Hcy,β_(2)-MG,and mAlb in combination in three subgroups based on AKI staging were higher than detection alone(P<0.01).ROC curve analysis showed that the area under the ROC curve(AUC)of combined detection of Hcy,β_(2)-MG,and mAlb in predicting AKI was larger,and the sensitivity and specificity were higher,as compared with that of detection alone.Conclusion The levels of Hcy,β_(2)-MG and mAlb are closely related to the onset and severity of AKI.Combined detection of Hcy,β_(2)-MG and mAlb has a higher predictive value for AKI.
