MSCT联合MRI对老年骨挫伤及隐匿性骨折的诊断价值

摘　　要：目的探讨多层螺旋CT(MSCT)联合MRI诊断老年骨挫伤及隐匿性骨折的价值。方法选取2021年6月—2022年6月诊治的120例疑似老年骨挫伤及隐匿性骨折,均行MSCT和MRI检查,以手术或关节镜检查结果为“金标准”,统计分析MSCT和MRI单独与联合诊断老年骨挫伤及隐匿性骨折的价值。结果经手术或关节镜检查结果显示,95例发生骨挫伤及隐匿性骨折,25例无骨挫伤及隐匿性骨折。MSCT联合MRI诊断老年骨挫伤及隐匿性骨折的敏感度、特异度、准确度均为100.00%,联合诊断的敏感度、准确度均高于MSCT单独诊断(P<0.01);2名经验丰富的影像学医师采用MSCT联合MRI诊断老年骨挫伤及隐匿性骨折的组内相关系数值为0.946(95%CI为0.857,0.976),具有较高一致性。MSCT联合MRI诊断不同类型老年骨挫伤及隐匿性骨折的准确度均为100.00%,高于MSCT单独诊断(P<0.01)。结论MSCT联合MRI诊断老年骨挫伤及隐匿性骨折的准确度高,可重复性强,具有较高临床应用价值。Objective To explore the value of multi-slice spiral CT(MSCT)combined with MRI in the diagnosis of bone contusion and occult fracture in the elderly.Methods A total of 120 elderly patients with suspected bone contusion and occult fracture treated from June 2021 to June 2022 were selected and underwent MSCT and MRI examination.With the results of surgery or arthroscopy as the"gold standard",the value of MSCT and MRI alone and in combination in the diagnosis of bone contusion and occult fracture in the elderly was statistically analyzed.Results The results of surgical or arthroscopic examination showed that 95 cases had bone contusion and occult fractures,and 25 cases had no bone contusions and occult fractures.The sensitivity,specificity and accuracy of MSCT combined with MRI in the diagnosis of bone contusion and occult fracture in the elderly were all 100.00%,and the sensitivity and accuracy of MSCT combined with MRI in the diagnosis were higher than that of MSCT alone(P<0.01).Two experienced radiologists used MSCT combined with MRI to diagnose bone contusion and occult fracture in the elderly,and the intragroup correlation value was 0.946(95%CI:0.857,0.976),showing high consistency.The accuracy of MSCT combined with MRI in the diagnosis of bone contusion and occult fracture of different types in the elderly was 100.00%,which was higher than that of MSCT alone(P<0.01).Conclusion MSCT combined with MRI has high accuracy and repeatability in the diagnosis of bone contusion and occult fracture in the elderly,and has high clinical application value.
