摘　　要：目的探讨瑞马唑仑联合舒芬太尼用于门诊老年患者无痛胃镜检查的安全性和有效性。方法回顾性分析2020年1—12月行无痛胃镜检查的老年患者120例,根据麻醉用药分为丙泊酚联合舒芬太尼组(P组)、瑞马唑仑联合舒芬太尼组(R组),每组60例。观察2组麻醉效果和镜检前后生命体征的变化,并观察围麻醉期不良事件的发生率。结果R组操作医师满意度高于P组,患者睫毛反射消失时间、清醒时间短于P组(P<0.05)。镜检后R组血氧饱和度(SpO_(2))、心率、平均动脉压高于P组(P<0.05,P<0.01);R组在镜检前后SpO_(2)、心率、平均动脉压差值小于P组(P<0.05,P<0.01)。R组围麻醉期心动过缓、呼吸抑制、低血压、注射痛、术中体动发生率低于P组(P<0.05,P<0.01)。结论瑞马唑仑联合舒芬太尼用于门诊老年患者无痛胃镜检查能够获得更好的镇静效果,对生命体征影响较小,能够降低不良事件发生率,安全性更高。Objective To discuss the safety and efficacy of Remimazolam combined with Sufentanil on painless gastroscopy in elderly patients.Methods A retrospective analysis was performed on 120 elderly patients who underwent painless gastroscopy from January to December 2020 and were divided into Propofol combined with Sufentanil group(group P)and Remazolam combined with Sufentanil group(group R)according to anesthetic drugs,with 60 cases in each group.The effects of anesthesia and the changes of vital signs before and after microscopic examination were observed,and the incidence of adverse events in perianaesthesia was observed.Results The satisfaction of the operator in group R was higher than that in group P,and the disappearance time of eyelash reflex and awake time after examination were shorter than those in group P(P<0.05).After microscopic examination,blood oxygen saturation(SpO_(2)),heart rate and mean arterial pressure in R group were higher than those in P group(P<0.05,P<0.01).SpO_(2),heart rate and mean arterial pressure in R group were lower than those in P group before and after microscopic examination(P<0.05,P<0.01).The incidence of bradycardia,respiratory depression,hypotension,injection pain in perianaesthesia and intraoperative motility in group R was lower than that in group P(P<0.05,P<0.01).Conclusion Remimazolam combined with Sufentanil on painless gastroscopy in elderly patients can achieve better sedation effect,has fewer impacts on vital signs,and reduce the incidence of adverse events,with better safety.
