摘 要:目的 探讨低浓度对比剂、低管电压与心电门控技术联合在胸腹主动脉CT血管成像(CTA)中的价值。方法 将我院2019年1月至2022年3月拟行胸腹主动脉CTA检查的100例男性患者依照检查先后顺序分为2组,即A组50例与B组50例,其中A组采用NI=11、管电压80 k V扫描条件和对比剂碘克沙醇(270 mgI/ml)和心电门控技术,重建算法采用自适应迭代技术(AIDR);B组患者采用NI=10、管电压120 kV和对比剂优维显(370 mgI/ml)和心电门控技术,重建算法为滤波反投影(FBP)技术。2组对比剂总量根据患者体质量1.3 ml/kg换算,最大为100 ml,注射流率依照实际情况设定,后续按相同速率注射0.9%氯化钠30 ml,对比2组辐射剂量、血管CT值、信噪比(SNR)、噪声比(CNR)以及图像质量。结果 2组年龄、体质量、BMI比较差异无统计学意义(P>0.05),对比剂注射流率、对比剂剂量比较(P>0.05);2组CTA影像均达到诊断标准差异无统计学意义(P>0.05);A组优良率(62%)优于B组(52%),但差异无统计学意义(P>0.05),A组合格率(94%)优于B组(76%)(P<0.05);A组与B组的竖脊肌CT值对比,差异无统计学意义(P>0.05),A组与B组主动脉CTA的CT值、CNR、SNR、SD对比差异有统计学意义(P均<0.05);A组的CT剂量指数(CTDI)vol、剂量长度乘积(DLP)和有效剂量(ED)均低于B组(P<0.05)。结论 将低管电压、低浓度对比剂及心电门控技术应用在胸腹主动脉CTA成像中,获得了较佳的效果,辐射剂量降低,并且图像质量处于较高水平,可满足诊断需求。对比剂的需求也得到了有效降低,对比剂浓度从370 mgI/ml降低至270 mgI/ml,使得患者的碘当量平均降低27.03%。Objective To explore the value of low concentration, low tube voltage contrast agent and ECG gating technique in CTA of thoracic and abdominal arteries. Methods One hundred male patients, proposed to undergo CTA of thoracic and abdominal aorta from January 2019 to March 2022, were divided into two groups according to the order of examination, That is, Group A(n=50) and Group B(n=50), In group A, NI=11, tube voltage80 kV scanning conditions and the contrast agent iodixadol(270 mgI/ml) and ECG gating technology,The reconstruction algorithm used adaptive iterative dose reduction(AIDR);In group B, NI=10, 120 k V, contrast contrast(370 mgI/ml) and ECG gating, The reconstruction algorithm is the filtered back-projection(FBP) technology.The total amount of contrast agent in the two groups was converted according to the patients′ body weight of 1.3 ml/kg, the maximum was 100ml, and the injection flow rate was set according to the actual situation. Subsequently, 30 ml of0.9% sodium chloride was injected at the same rate. The radiation dose, vascular CT value, signal to noise ratio(SNR), noise ratio(CNR) and image quality of the two groups and groups were compared. Results Comparison of age, body quality, and BMI( P>0.05), Comparison of contrast agent injection flow rate and contrast agent dose( P>0.05);The CTA imaging in all 2 groups met the diagnostic criteria(P>0.05);The excellent rate in group A was 62%,which was better than 52% in group B, However, the difference was not significant(P>0.05), The qualified rate of group A was 94%, which was better than 76% of group B(P<0.05);Erector spine muscle CT values comparison between group A and group B, The difference was not significant(P>0.05), The CT values, CNR, SNR, and SD comparison of aortic CTA between group A and group B(P<0.05);CTDIvol, DLP and ED were lower in group A than in group B(P<0.05).
