摘　　要：目的:探讨Brostr(o)m术式对慢性踝关节外侧副韧带损伤的临床疗效及对疼痛的影响。方法:将我院2017-06~2020-06收治的90例行手术治疗的慢性踝关节外侧副韧带损伤患者,按手术方式分为A组和B组,各45例。A组给予解剖修复术,B组给予Brostr(o)m术,术后随访12个月。比较两组患者手术指标,术后8周、3个月、6个月、12个月踝-后足(AOFAS)评分与疼痛评分以及不良反应发生情况。结果:两组患者手术时间差异无统计学意义(P>0.05),B组切口长度短于A组,住院天数短于A组(P<0.05),与术前相比,术后8周、3个月、6个月、12个月AOFAS评分均上升,两组术后8周、3个月、6个月、12个月AOFAS评分差异均有统计学意义(P<0.05),与术前相比,术后8周、3个月、6个月、12个月疼痛评分均降低,两组术后8周、3个月、6个月、12个月疼痛评分差异均有统计学意义(P<0.05),两组患者并发症发生率差异无统计学意义(P>0.05)。结论:Brostr(o)m术式可有效改善慢性踝关节外侧副韧带损伤患者踝关节功能,减小切口长度,缩短住院时间,降低疼痛水平,安全性较高且疗效确切。Objective:To investigate the clinical effect of brostr(o)m operation on chronic lateral collateral ligament injury of ankle and its effect on pain.Methods:90 patients with chronic lateral collateral ligament injury of ankle treated surgically in our hospital from June 2017 to June 2020 were divided into group A and group B,with 45 cases in each group.Group A was treated with anatomical repair and group B was treated with brostr(o)M.the patients were followed up for 12 months.The operative indexes,ankle hindfoot(AOFAS)score,pain score and adverse reactions were compared between the two groups at 8 weeks,3 months,6 months and 12 months after operation.Result:There was no significant difference in operation time between the two groups(P>0.05).The incision length in group B was lower than that in group A,and the length of hospital stay was shorter than that in group A(P<0.05).Compared with before operation,the AOFAS scores increased at 8 weeks,3 months,6 months and 12 months after operation.There were significant differences in AOFAS scores at 8 weeks,3 months,6 months and 12 months after operation between the two groups(P<0.05)The pain scores decreased at 6 and 12 months.There was significant difference in the pain scores at 8 weeks,3 months,6 months and 12 months between the two groups(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Brostr(o)m operation can effectively improve the surgical indexes and ankle function of patients with chronic lateral collateral ligament injury of ankle,reduce the level of pain,have high safety and definite curative effect.
