不同免疫状态隐球菌性脑膜炎患者的外周血淋巴细胞亚群比较

摘　　要：目的:探讨不同免疫状态的隐球菌性脑膜炎患者的外周血淋巴细胞亚群比较。方法:将我院2019-03~2021-02收治的122例隐球菌性脑膜炎患者,根据免疫状态分为无免疫抑制组和免疫抑制组,各61例,另选取50例同期在我院进行体检的健康人群作为对照组。比较3组研究对象B淋巴细胞、外周血淋巴细胞、CD_(3)^(+)T淋巴细胞、CD_(3)^(+)CD_(4)^(+)T淋巴细胞、CD_(3)^(+)CD_(8)^(+)T淋巴细胞、CD_(4)^(+)/CD_(8)^(+)T淋巴细胞比值,分析隐球菌性脑膜炎患者脑脊液检查结果。结果:无免疫抑制组和免疫抑制组B淋巴细胞、CD_(3)^(+)T淋巴细胞、CD_(3)^(+)CD_(4)^(+)T淋巴细胞计数低于对照组,免疫抑制组B淋巴细胞、CD_(3)^(+)T淋巴细胞、CD_(3)^(+)CD_(4)^(+)T淋巴细胞计数低于无免疫抑制组(P<0.05),3组NK淋巴细胞、CD_(3)^(+)CD_(8)^(+)T淋巴细胞、CD_(4)^(+)/CD_(8)^(+)T淋巴细胞比值差异无统计学意义(P>0.05);122例隐球菌性脑膜炎患者脑脊液涂片阳性为多见,占比90.16%,脑脊液压力>200mmH_(2)O占比最多50.82%,脑脊液蛋白≥450 mg/L占比最多82.79%,脑脊液葡萄糖<2.5mmol/L占比最多65.57%,脑脊液细胞数>100×10^(5)/L占比最多50.82%。结论:隐球菌性脑膜炎患者,无论是否存在免疫抑制基础疾病,外周血淋巴细胞亚群水平均较低。Objective:To investigate the comparison of peripheral blood lymphocyte subsets in patients with cryptococcal meningitis in different immune states.Methods:122 patients with cryptococcal meningitis treated in our hospital from March 2019 to February 2021 were divided into non immunosuppressive group and immunosuppressive group,61 cases in each group,and 50 healthy people who underwent physical examination in our hospital at the same time were selected as the control group.The ratios of B lymphocytes,peripheral blood lymphocytes,CD_(3)^(+)T lymphocytes,CD_(3)^(+)CD_(4)^(+)T lymphocytes,CD_(3)^(+)CD_(8)^(+)T lymphocytes and CD_(4)^(+)/CD_(8)^(+)T lymphocytes in the three groups were compared,and to analyze the results of cerebrospinal fluid examination in patients with cryptococcal meningitis.Result:The counts of B lymphocytes,CD_(3)^(+)T lymphocytes and CD_(3)^(+)CD_(4)^(+)T lymphocytes in non immunosuppression group and immunosuppression group were lower than those in control group,the counts of B lymphocytes,CD_(3)^(+)T lymphocytes and CD_(3)^(+)CD_(4)^(+)T lymphocytes in immunosuppression group were lower than those in non immunosuppression group(P<0.05),NK lymphocytes,CD_(3)^(+)CD_(8)^(+)Tlymphocytes There was no significant difference in CD_(4)^(+)/CD_(8)^(+)T lymphocyte ratio(P>0.05).Positive cerebrospinal fluid smears were common in 122 patients with cryptococcal meningitis,accounting for 90.16%,cerebrospinal fluid pressure>200mm H_(2)O accounting for 50.82%,cerebrospinal fluid protein≥450mg/L accounting for 82.79%,cerebrospinal fluid glucose<2.5mmol/L accounting for 65.57%,and cerebrospinal fluid cell number>100×10^(5)/L accounting for 50.82%.Conclusion:In patients with cryptococcal meningitis,the level of peripheral blood lymphocyte subsets was low whether there were immunosuppressive basic diseases or not.
