摘　　要：目的:分析急性毒蕈中毒患者的临床特征,并调查中毒死亡原因。方法:选取2017-04~2020-04在本院进行治疗急性毒蕈中毒患者84例,死亡11例,存活73例,分析患者中毒后的临床表现特征,并采用多因素Logistic回归分析法分析急性毒蕈中毒患者死亡的独立危险因素。结果:84例患者中11例死亡,死亡率13.10%;大部分患者表现出明显的胃肠道症状,并出现呕吐和腹泻;毒性作用主要累及肝脏,并出现神经系统和心脏功能损伤。经多因素Logistic回归分析法显示发病至入院时间≥6h、谷丙转氨酶(ALT)≥200U/L、谷草转氨酶(AST)≥200U/L及活化部分凝血活酶时间(APTT)≥40s等为导致患者死亡的独立危险因素。结论:病程≥6h、谷丙转氨酶(ALT)≥200U/L、谷草转氨酶(AST)≥200U/L及活化部分凝血活酶时间(APTT)≥40 s等均是导致急性毒蕈中毒患者死亡的重要原因,且肝脏为受损的重要器官,临床治疗和护理应提高对上述因素的重视,提高医疗、护理技术的同时,减少患者死亡率。Objective:To analyze the clinical characteristics of patients with acute mushroom poisoning and investigate the cause of death due to poisoning.Methods:~April 2017~April 2020 treated 84 patients,11 dead,73 alive,analyzed the clinical presentation characteristics of patients after poisoning and analyzed independent risk factors for death by multivariate Logistic regression analysis.Results:Eleven of the 84 patients died,with a mortality rate of 13.10%;most patients showed significant gastrointestinal symptoms with vomiting and diarrhea;toxic effects mainly involved the liver and impairment in neurological and cardiac function.Multivariate Logistic regression analysis showed≥onset to 6h,alanine transaminase(ALT)≥200U/L,aspartate transaminase(AST)≥200U/L and activated partial live thrombin time(APTT)≥40s as independent risk factors for patient death.Conclusion:The course of disease≥6h,alanine aminotransferase(ALT)≥200U/L,aspartate aminotransferase(AST)≥200U/L and activated partial thromboplastin time(APTT)≥40s and so on are the important causes of death in patients with acute mushroom poisoning,and liver is an important organ damaged.Clinical treatment and nursing should pay more attention to the above factors,improve medical and nursing technology,and reduce the mortality of patients.
