摘 要:目的研究超声测量健康儿童腭扁桃体大小、弹性及其与个体因素的相关性,为进一步探究生物组织弹性与免疫功能的关系提供多参数支持。资料与方法前瞻性选取2019年5月—2020年12月224名于河南中医药大学第一附属医院门诊体检的健康儿童,根据年龄分为1~4岁、5~7岁、8~10岁、11~13岁、14~16岁组,记录性别、年龄、身高、体重及体重指数(BMI)等。全部入组儿童均采用经颈部超声测量双侧腭扁桃体各径线并计算其体积;采用声触诊组织定量技术测量其双侧腭扁桃体腺体的剪切波速度(SWV),并评估组织弹性。比较各年龄组双侧腭扁桃体各径线、体积及SWV。分析腭扁桃体体积及SWV与身高、体重及BMI的相关性。结果5组健康儿童腭扁桃体体积及SWV随年龄增长而增大。双侧腭扁桃体的中位前后径为15.13(11.85,16.95)mm;中位左右径为13.33(9.65,15.35)mm;中位上下径为14.58(10.75,17.25)mm。所有纳入儿童的双侧腭扁桃体中位体积为1.54(0.67,2.26)cm^(3);双侧腭扁桃体中位SWV为1.27(0.72,1.57)m/s。健康儿童腭扁桃体体积与身高(r=0.92)、体重(r=0.89)及BMI(r=0.54)呈正相关(均P<0.01)。不同性别健康儿童腭扁桃体间SWV比较,差异有统计学意义(Z=-2.63,P<0.05)。结论超声对于评估腭扁桃体体积及组织弹性具有重要意义。16岁以下儿童腭扁桃体体积随年龄增长而增大,且组织弹性逐渐降低。腭扁桃体各径线及体积与身高、体重和BMI有关。腭扁桃体腺体组织弹性与身高和BMI具有相关性。Purpose To study the size and elasticity of palatine tonsils measured by ultrasound in healthy children and their correlation with individual factors,providing multi-parameter support for further exploring the relationship between biological tissue elasticity and immune function.Materials and Methods A total of 224 healthy children who underwent outpatient/physical examination in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from May 2019 to December 2020 were selected prospectively as the research objects.They were divided into five subgroups:1-4 years old group;5-7 years old group;8-10 years old group;11-13 years old group;14-16 years old group according to different ages.Gender,age,height,weight and body mass index(BMI)were recorded.All the patients were measured by transcervical ultrasound,and the shear wave velocity(SWV)of bilateral palatal tonsils was measured by virtual touch tissue quantification.The diameters,volumes and SWV of bilateral palatal tonsils in different age groups were compared,and the possible correlations between palatal tonsil volume and SWV with height,weight and BMI were analyzed.Results The volume of palatine tonsils and SWV of healthy children in five subgroups increased with age.The median anterior posterior diameter of bilateral palatine tonsils was15.13(11.85,16.95)mm.The median left and right diameter was 13.33(9.65,15.35)mm.The median upper and lower diameter was 14.58(10.75,17.25)mm.The median volume of bilateral palatine tonsils in all included children was 1.54(0.67,2.26)cm^(3).The median SWV of bilateral palatine tonsils was 1.27(0.72,1.57)m/s.In healthy children,the volume of palatine tonsils was positively correlated with height(r=0.92,P<0.01),body weight(r=0.89,P<0.01)and BMI(r=0.54,P<0.01).SWV comparison between different sex palatine tonsil healthy children,the difference was statistically significant(Z=-2.63,P<0.05).Conclusion Ultrasound is of great significance to evaluate the volume and tissue elasticity of palatine tonsils.In child
