摘  要:目的探讨医务人员接种新型冠状病毒(SARS-CoV-2)灭活疫苗后,生化指标、中和抗体、IgM和IgG抗体在45 d内的变化情况。方法收集2021年9月15日至11月2日期间,在该院接种SARS-CoV-2疫苗的102例医护人员的血清样本,对疫苗接种前后生化指标和抗体的变化结果进行差异性分析。结果疫苗接种前和接种后45 d,丙氨酸氨基转移酶、谷氨酸氨基转移酶、肌酸激酶、乳酸脱氢酶、尿素氮、肌酐指标比较差异无统计学意义(P>0.05),总蛋白(TP)、白蛋白(ALB)有轻微升高(P<0.05),但变化都在正常参考范围以内。接种疫苗后45 d,ALB、TP的曲线下面积(AUC)分别为(0.988、0.975,P<0.05),ALB、TP灵敏度均为100%,特异度分别为87.25%、87.39%;在疫苗接种后15 d,IgG抗体、IgM抗体阳性率差异无统计学意义(χ^(2)=0.049,P>0.05);在疫苗接种后30 d,中和抗体与IgG抗体、IgM抗体阳性率差异有统计学意义(χ^(2)=7.249、46.295,P<0.05);在疫苗接种后45 d中和抗体阳性率对比IgG抗体阳性率无统计学意义(χ^(2)=3.045,P>0.05);中和抗体在苗接种后30、45 d阳性率对比差异无统计学意义(χ^(2)=1.682,P>0.05);IgM抗体阳性率随时间增长下降较快,同期的IgG抗体阳性率一直在100%的高位水平,中和抗体阳性率随着接种疫苗后时间的延长一直呈上升趋势。结论结论ALB和TP具有较高的评估效能,疫苗接种前和疫苗接种后45 d,所有生化项目指标变化都在正常参考范围内,疫苗接种后45 d三种抗体有各自不同的变化规律。Objective To investigate the changes of biochemical indexes,neutralizing antibodies,IgM and IgG antibodies within 45 days after medical staff inoculated with the novel coronavirus(SARS-CoV-2)inactivated vaccine.Methods The serum samples of 102 medical staff who were vaccinated with SARS-CoV-2 vaccine in the hospital from September 15 to November 2,2021 were collected,and the results of changes in biochemical indicators and antibodies before and after vaccination were compared.analyze.Results Before and after vaccination,there was no significant difference in ALT,AST,CK,LDH,BUN and CREA(P>0.05),while ALB and TP were slightly increased(P<0.05),but the changes were normal.within the reference range.At 45 days after vaccination,the area under the curve(AUC)of ALB and TP were(0.988,0.975,P<0.05),the sensitivity of ALB and TP were 100%,and the specificity was 87.25% and 87.39%,respectively.After 15 days,there was no significant difference in the positive rate of IgG antibody and IgM antibody(χ^(2)=0.049,P>0.05);30 days after vaccination,the difference between the positive rate of neutralizing antibody and IgG antibody and IgM antibody was statistically significant(χ^(2)=7.249,46.295,P<0.05);there was no significant difference in the positive rate of neutralizing antibody compared with the positive rate of IgG antibody at 45 days after vaccination(χ^(2)=3.045,P>0.05);neutralizing antibody at 30 days after vaccination Compared with the positive rate of 45 d,there was no statistical significance(χ^(2)=1.682,P>0.05);the positive rate of IgM antibody decreased rapidly with time,and the positive rate of IgG antibody in the same period was always at a high level of 100%,and the positive rate of neutralizing antibody was positive.The rate has been increasing with time since vaccination.Conclusion ALB and TP have high evaluation performance.Before vaccination and 45 days after vaccination,the changes of all biochemical indicators are within the normal reference range,and the three antibodies have different changing rules at
