改良型通脑活络针刺法在治疗脑卒中并发假性球麻痹所致吞咽障碍患者中的疗效观察

摘　　要：目的探讨改良型通脑活络针刺法在脑卒中后并发假性球麻痹所致吞咽障碍患者康复中的疗效。方法选取2020年4月至2021年12月在南京医科大学附属无锡人民医院针灸科治疗的脑卒中后并发假性球麻痹所致吞咽障碍患者72例,采用随机数表法分为治疗组和对照组,每组各36例。治疗组运用改良型通脑活络针刺法,对照组运用常规阳明经针刺法。两组治疗10次后,观察治疗前后洼田饮水试验(WST)、标准吞咽功能评定量表(SSA)、吞咽障碍的结局和严重度量表(DOSS)的指数变化及临床疗效。结果治疗后,两组WST、SSA、DOSS指数均较治疗前改善,且治疗组改善更明显,差异有统计学意义(P<0.05)。与对照组比较,治疗组的总有效率更高,差异有统计学意义(P<0.05)。结论改良型通脑活络针刺法在脑卒中后并发假性球麻痹所致吞咽障碍患者康复中确有疗效,值得推广。Objective To explore the efficacy of modified Tongnao Huoluo acupuncture in the rehabilitation of dysphagia caused by pseudobulbar paralysis after stroke.Methods A total of 72 patients with dysphagia caused by pseudobulbar paralysis after stroke admitted to the Department of Acupuncture and Moxibustion of Wuxi People’s Hospital Affiliated to Nanjing Medical University from April 2020 to December 2021 were selected and divided into the treatment group and the control group according to the random number table method,with 36 cases in each group.The treatment group was treated with the modified Tongnao Huoluo acupuncture,and the control group was treated with the routine foot-Yangming Meridian acupuncture.After 10 treatment courses in two groups,the index changes of water swallow test(WST),standardized swallowing assessment(SSA),dysphagia outcome and severity scale(DOSS)were observed before and after treatment,and observe the clinical efficacy.Results After treatment,the indexes of WST,SSA,DOSS in two groups were improved compared with those before treatment,and the improvement was more significant in the treatment group,with statistically significant differences(P<0.05).The overall efficacy was higher in the treatment group than that in the control group,with statistically significant differences(P<0.05).Conclusion The modified Tongnao Huoluo acupuncture is effective in the rehabilitation of dysphagia caused by pseudobulbar paralysis after stroke,which is worth popularizing.
