摘 要:目的检测并分析血清免疫细胞因子在非结核分枝杆菌肺病(NTM-PD)和继发性肺结核(PTB)鉴别中的应用。方法前瞻性选取2021年7—12月于广州市胸科医院就诊的PTB患者和NTM-PD患者作为研究对象,分为PTB组和NTM组。其中NTM组全部入选,PTB组采取等距抽样,PTB组患者39例,NTM组患者41例。采用酶联免疫吸附剂测定(ELISA)方法检测血清中免疫相关因子白介素-2(IL-2)、白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)的水平,并对数据进行统计学分析。结果NTM组患者IL-8浓度显著高于PTB组,IL-2浓度显著低于NTM组,差异有统计学意义(P<0.05);两组患者IL-6、IL-10和TNF-α浓度比较,差异无统计学意义(P>0.05)。进行ROC曲线分析,IL-2、IL-8和IL-10三种细胞因子联合检测的曲线下面积大于任一单个细胞因子检测的曲线下面积(P<0.05)。结论IL-2、IL-8和IL-10联合检测在临床诊断鉴别PTB和NTM-PD有临床价值。Objective To detect and analyze the application of serum immune cytokines in the differentiation of secondary pulmonary tuberculosis(PTB)and non-pulmonary mycobacterium tuberculosis(NTMPD).Methods Patients with PTB and NTM-PD admitted to our hospital from July to Deceber 2021 were prospectively selected as the study subjects,and were divided into PTB group and NTM group.All cases in the NTM group were enrolled,and isometric sampling was used in the PTB group.There were 39 cases in PTB group and 41 cases in NTM group.Serum levels of immune-related factors interleukin-2(IL-2),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α)were detected by enzyme linked immunosorbent assay(ELISA),and the data were statistically analyzed.Results Compared with PTB group,serum IL-8 was significantly increased while IL-2 was significantly decreased in NTM group,the differences were statistically significant(P<0.05);the concentration of IL-6,IL-10 and TNF-αwere compared between the two groups,but the differences were not statistically significant(P>0.05);ROC curve analysis showed that the area under curve of the combined detection of IL-2,IL-8 and IL-10 was larger than that of any single cytokine(P<0.05).Conclusion The combined detection of IL-2,IL-8 and IL-10 has clinical value in differentiating PTB from NTM-PD.
