老年冠心病并发巨细胞病毒感染与血液流变学和内皮素水平及预后的关系

摘　　要：目的 探究老年冠心病患者巨细胞病毒(CMV)感染血液流变学和内皮素水平及其预后。方法 选取2017年10月-2019年6月河南科技大学第二附属医院收治的老年冠心病CMV感染患者86例为感染组,CMV未感染患者71例为非感染组,比较两组血液流变学指标、内皮素水平及心血管不良事件发生率,分析CMV-DNA载量与血液流变学指标及内皮素水平的关系。结果 感染组高切黏度(6.89±0.95)mPa·s、低切黏度(11.12±1.20)mPa·s、血浆黏度(2.31±0.45)mPa·s、红细胞压积(53.54±4.97)%、内皮素水平(89.34±9.63)pg/ml均高于非感染组(P<0.05);CMV-DNA载量≥1×10^(3)拷贝/ml患者的内皮素水平高于CMV-DNA载量<1×10^(3)拷贝/ml患者(P<0.05);感染组心血管不良事件发生率为12.79%高于非感染组(P<0.05)。结论 CMV感染会增加老年冠心病患者血液流变学指标及内皮素水平,并增加老年冠心病心血管不良事件的发生。OBJECTIVE To explore the association of cytomegalovirus(CMV) infection with hemorheology, endothelin level and prognosis of elderly patients with coronary heart disease. METHODS A total of 86 elderly patients with coronary heart disease who had CMV infection and were treated in the Second Affiliated Hospital of Henan University of Science and Technology from Oct 2017 to Jun 2019 were assigned as the infection group, and 71 patients without CMV infection were chosen as the no infection group. The hemorheological indexes, endothelin level and incidence of adverse cardiovascular events were compared between the two groups, and the relationship between CMV-DNA load and hemorheological indexes, endothelin level was observed. RESULTS The high shear viscosity, low shear viscosity, plasma viscosity, Hematocrit and endothelin level of the infection group were respectively(6.89±0.95)mPa·s,(11.12±1.20)mPa·s,(2.31±0.45)mPa·s, (53.54±4.97)% and(89.34±9.63)pg/ml, significantly higher than those of the no infection group(P<0.05). The endothelin level of the patients with CMV-DNA load no less than 1×10^(3) copies/ml was significantly higher than that of the patients with CMV-DNA load less than 1×10^(3) copies/ml(P<0.05). The incidence of adverse cardiovascular events of the infection group was 12.79%, significantly higher than that of the no infection group(P<0.05). CONCLUSION The CMV infection may raise the levels of hemorheological indexes and endothelin of the elderly patients with coronary heart disease and lead to the increase of incidence of adverse cardiovascular events.
