老年高血压并发Hp感染对糖脂代谢和颈动脉血流动力学的影响

摘　　要：目的探讨高龄高血压患者幽门螺杆菌(Hp)感染情况及其对糖脂代谢、颈动脉形态和血流动力学的影响。方法选取2018年5月-2020年5月于天津医院健康管理科体检结果为高血压且年龄>60岁患者146例,根据Hp感染情况分为Hp感染组69例和非Hp感染组77例,比较两组患者外周血糖代谢相关指标水平、血脂水平,颈动脉超声观察两组患者颈动脉形态和血流动力学差异。结果Hp感染组空腹和服糖后0.5、1、2、3 h胰高血糖素(GLuca)水平均高于非Hp感染组(P<0.05),生长抑素(SS)水平均低于非Hp感染组(P<0.05);Hp感染组患者服糖后1、2、3 h胰岛素(INS)水平均高于非Hp感染组(P<0.05)。Hp感染组患者的三酰甘油(TG)和低密度脂蛋白胆固醇(LDL-C)水平均高于非Hp感染组(P<0.05),高密度脂蛋白胆固醇(HDL-C)水平低于非Hp感染组(P<0.05);Hp感染组患者的颈动脉内膜中层厚度(IMT)、颈动脉内径和搏动指数均高于非Hp感染组(P<0.05),收缩期峰值流速(PSV)、舒张末期流速(EDV)和最大剪切率均低于非Hp感染组(P<0.05)。结论Hp感染可使高血压患者糖脂代谢和颈动脉形态发生改变,并影响颈动脉血流动力学,促进心脑血管疾病发生。OBJECTIVE To explore the prevalence of Helicobacter pylori(Hp)infection and its influence on glucose-lipid metabolism,carotid morphology and hemodynamics of elderly patients with hypertension.METHODS A total of 146 patients with more than 60 years of age who received physical examination and were diagnosed with hypertension in health management department of Tianjin Hospital from May 2018 to May 2020 were enrolled in the study and divided into the Hp infection group with 69 cases and the no Hp infection group with 77 cases according to the status of Hp infection.The levels of peripheral blood glucose metabolism-related indexes and blood lipid level were compared between the two groups of patients,the carotid morphology and hemodynamics were observed and compared between the two groups of patients by carotid artery ultrasound.RESULTS The fasting glucagon(GLuca)level and GLuca levels after glucose supplement for 0.5,1,2 and 3 hours were significantly higher in the Hp infection group than in the no Hp infection group(P<0.05);the somatostatin(SS)level of the infection group was significantly lower than that of the no Hp infection group(P<0.05);the insulin(INS)level of the Hp infection group was significantly higher than that of the no Hp infection group after glucose supplement for 1,2 and 3 hours(P<0.05).The levels of triacylglycerol(TG)and low density lipoprotein cholesterol(LDL-C)of the Hp infection group were significantly higher than those of the no Hp infection group(P<0.05),the high density lipoprotein cholesterol(HDL-C)of the Hp infection group was significantly lower than that of the no Hp infection group(P<0.05).The carotid intima-media thickness(IMT),carotid diameter and pulsatility index of the Hp infection group were significantly higher than those of the no Hp infection group(P<0.05);the peak systolic velocity(PSV),end-diastolic velocity(EDV)and maximum shear rate were significantly lower in the Hp infection group than in the no Hp infection grou(P<0.05).CONCLUSION The Hp infection may alter the glucose
