糖尿病足感染血清IFN-γ、Cys-C、FIB水平与感染严重程度和预后的关系

摘　　要：目的探究糖尿病足感染患者血清干扰素γ(IFN-γ)、半胱氨酸蛋白酶抑制剂C(Cys-C)及纤维蛋白原(FIB)水平与感染严重程度和预后的关系。方法选取2019年1月-2020年12月天津市第五中心医院诊治的糖尿病足患者124例,根据是否感染分为感染组60例和非感染组64例,分析糖尿病足感染血清IFN-γ、Cys-C及FIB水平变化及其对治疗效果的预测价值。结果感染组IFN-γ、Cys-C、FIB水平较非感染组升高,且随着感染严重程度的增加,水平逐渐升高(P<0.05);治疗有效组血清IFN-γ、Cys-C、FIB下降程度优于无效组(P<0.05);血清IFN-γ、Cys-C、FIB水平联合预测糖尿病足患者治疗无效的曲线下面积(AUC)为0.852、敏感度为82.47%、特异度为77.78%。结论糖尿病足感染患者血清IFN-γ、Cys-C、FIB水平升高,且与患者感染严重程度相关,三项指标联合检测对糖尿病足治疗效果有一定的预测价值。OBJECTIVE To explore the relationship between the levels of serum interferon-γ(IFN-γ),cysteine C(Cys-C)and fibrinogen(FIB)and the severity of infection and prognosis of the patients with diabetic foot infection.METHODS A total of 124 diabetic foot patients who were treated in Tianjin Fifth Central Hospital from Jan 2019 to Dec 2020 were enrolled in the study and divided into the infection group with 60 cases and the no infection group with 64 cases according to the status of infection.The changes of levels of serum IFN-γ,Cys-C and FIB in the patients with diabetic foot infection were observed,and the values of the above indexes in prediction of curative effects were analyzed.RESULTS The levels of IFN-γ,Cys-C and FIB of the infection group were significantly higher than those of the no infection group,which were gradually elevated with the increase of severity of infection(P<0.05);the levels of serum IFN-γ,Cys-C and FIB declined more significantly in the effective treatment group than in the ineffective treatment group(P<0.05).The area under curve(AUC)of the joint detection of serum IFN-γ,Cys-C and FIB was 0.852 in prediction of ineffective treatment of the diabetic foot patients,with the sensitivity 82.47%,the specificity 77.78%.CONCLUSION The levels of serum IFN-γ,Cys-C and FIB of the patients with diabetic foot infection are elevated,which is associated with the severity of infection.The joint detection of the three indexes has certain value in prediction of curative effect.
