冠心病支架植入术后肺部感染与Taql基因多态性及MMP-9和Ⅳ-C表达的关联

摘　　要：目的探究冠心病患者支架植入术后肺部感染与维生素D受体(VDR)Taql基因单核苷酸多态性及基质金属蛋白酶-9(MMP-9)和Ⅳ型胶原(Ⅳ-C)表达的相关性。方法选取2017年12月-2020年12月西安国际医学中心医院心脏内科收治的冠心病支架植入术患者为研究对象,其中选取术后并发肺部感染患者54例为感染组,未合并肺部感染患者164例为非感染组。聚合酶链反应-限制性片段长度多态性(PCR-RFLP)检测VDR基因Taql基因单核苷酸多态性,酶联免疫吸附法(ELISA)检测血清MMP-9和Ⅳ-C水平。结果感染组和非感染组患者VDR基因Taql基因rs731236位点基因型符合Hardy Weinberg检验,感染组t等位基因频率高于非感染组(P<0.05);感染组血清MMP-9和Ⅳ-C水平分别为(1.65±0.49)ng/L和(92.38±12.07)ng/ml高于非感染组(P<0.05);感染组不同基因型MMP-9、Ⅳ-C水平比较,差异有统计学意义(P<0.05),其中tt基因型水平最高。结论冠心病患者支架植入术后合并肺部感染与VDR基因Taql基因单核苷酸多态性有关,感染后患者血清MMP-9、Ⅳ-C水平升高。OBJECTIVE To explore the correlation between postoperative pulmonary infection in coronary heart disease(CHD)patients undergoing stent implantation and vtamin D receptor(VDR)Taql gene single nucleotide polymorphism and expression of matrix metalloproteinase-9(MMP-9)and Collagen type IV(IV-C).METHODS The CHD patients who underwent stent implantation and were treated in cardiology department of Xi′an International Medical Center Hospital from Dec 2017 to Dec 2020 were recruited as the study objects,54 patients who had postoperative pulmonary infection were assigned as the infection group,and 164 patients who were not complicated with pulmonary infection were assigned as the no infection group.The single nucleotide polymorphisms of VDR gene Taq1 were detected by polymerase chain reaction-restricted fragment length polymorphism(PCR-RFLP),and the levels of serum MMP-9 andⅣ-C were detected by means of enzyme-linked immunosorbent assay(ELISA).RESULTS The genotypes at rs731236 locus of VDR gene Taq1 of the patients in the infection group and the no infection group conformed to Hardy Weinberg test.The frequency of t allele of the infection group was significantly higher than that of the no infection group(P<0.05).The levels of serum MMP-9 andⅣ-C of the infection group were respectively(1.65±0.49)ng/L and(92.38±12.07)ng/ml,significantly higher than those of the no infection group(P<0.05);there were significant differences in the levels of MMP-9 andⅣ-C among the patients with different genotypes in the infection group(P<0.05),and the levels of MMP-9 andⅣ-C of the patients with tt genotype were the highest.CONCLUSION The postoperative pulmonary infection in the CHD patients undergoing stent implantation is associated with the single nucleotide polymorphisms of VDR gene Taq1,and the levels of serum MMP-9 andⅣ-C are elevated in the patients with infection.
