摘　　要：目的分析三磷酸腺苷结合盒转运子B亚家族成员1(ABCB1)基因多态性与糖皮质激素性股骨头坏死(SANFH)的关联性。方法选取2017年01月-2019年01月厦门大学附属第一医院收治的长期使用糖皮质激素治疗的95例患者,其中发生股骨头坏死(ONFH)患者55例为坏死组,未发生ONFH患者40例为对照组,检测两组骨代谢指标[Ⅱ型胶原羧基端端肽(CTXⅡ)、抗酒石酸酸性磷酸酶5b(TRACP-5b)],血流流变学指标(血黏度高切、血黏度低切、血浆黏度及红细胞压积);采用Sanger测序法检测ABCB1基因C1236T、G2677T/A及C3435T位点单核苷酸多态性(SNPs)。结果坏死组CTXⅡ(209.51±10.37)ng/L、TRACP-5b(52.04±6.55)U/L水平及血黏度高切(5.08±0.77)、血黏度低切(11.38±2.74)、血浆黏度(2.23±0.30)mPa·s及红细胞压积(50.92±7.12)%均高于对照组(P<0.05);坏死组ABCB1基因C3435T位点CC基因型及C等位基因频率高于对照组(P<0.05)。结论糖皮质激素性股骨头坏死患者骨代谢和血流流变学指标水平升高,ABCB1 C3435T位点C等位基因与SANFH易感性有关。OBJECTIVE To analyze the association of adenosine triphosphate-binding cassette transporter B subfamily member 1(ABCB1)gene polymorphism with glucocorticoid-induced osteonecrosis of the femoral head(SANFH).METHODS A total of 95 patients who were given glucocorticoids for a long time period and were treated in The First Affiliated Hospital of Xiamen University from Jan 20117 to Jan 2019 were enrolled in the study,55 of whom had osteonecrosis of femoral head(ONFH)and were assigned as the ONFH group,and 40 did not have ONFH and were assigned as the control group.The bone metabolism indexes[type II collagen carboxy-terminal peptide(CTXII),tartrate-resistant acid phosphatase 5 b(TRACP-5 b)]and hemorheological indexes(blood viscosity high shear,blood viscosity low shear,plasma viscosity and hematocrit)were detected,and the single nucleotide polymorphisms(SNPs)at C1236 T,G2677 T/A and C3435 T loci of ABCB1 gene were detected by Sanger sequencing method.RESULTS The CTXⅡlevel,TRACP-5 b level,high viscosity,low viscosity,plasma viscosity and hematocrit of the ONFH group were respectively(209.51±10.37)ng/L,(52.04±6.55)U/L,(5.08±0.77),(11.38±2.74),(2.23±0.30)mPa·s and(50.92±7.12)%,significantly higher than those of the control group(P<0.05).The frequencies of CC genotype and C allele at C3435 T of ABCB1 gene were significantly higher in the ONFH group than in the control group(P<0.05).CONCLUSION The levels of bone metabolism indexes and hemorheological indexes of the SANFH patients are elevated,and the C allele at C3435 locus of ABCB1 gene is associated with the susceptibility to SANFH.
